目的:探讨哮喘控制测试(asthma control test,ACT)评分与非过敏性哮喘患者诱导痰液中超敏 C 反应蛋白(high-sensitivity C-reactive protein,hs-CRP)及气道反应性的相关性。方法选择135例非过敏性哮喘患者(非过敏性哮喘组)及50例同期健康体检者(对照组),检测2组痰液中 hs-CRP 含量及肺功能情况。再依据入院时ACT 评分将非过敏性哮喘患者分为2组,A 组58例(ACT 评分≥20),B 组77例(ACT 评分<20),并检测其气道反应性。采用 Pearson 相关检验分析 ACT 评分与 hs-CRP 及气道反应性的相关性。结果非过敏性哮喘患者痰液中 hs-CRP 含量明显高于对照组,而肺功能各指标 FEV1、FEV1%、PEF 及 PEF%则显著低于对照组(均 P <0.01)。非过敏性哮喘 B 组患者痰液中 hs-CRP 水平较 A 组高(P <0.01),而反映气道反应性的 FEV1改变量均显著低于 A 组(P <0.05)。ACT 评分与 hs-CRP 含量呈负相关(r =-0.76,P <0.01),与 FEV1增加程度(%)和绝对值增加量(mL)呈正相关(r=0.85,P <0.05;r =0.916,P <0.05)。结论ACT 评分与非过敏性哮喘患者升高的 hs-CRP 水平及气道反应性密切相关,正确使用该评分有助于简便、快捷地进行病情评估。

목적:탐토효천공제측시(asthma control test,ACT)평분여비과민성효천환자유도담액중초민 C 반응단백(high-sensitivity C-reactive protein,hs-CRP)급기도반응성적상관성。방법선택135례비과민성효천환자(비과민성효천조)급50례동기건강체검자(대조조),검측2조담액중 hs-CRP 함량급폐공능정황。재의거입원시ACT 평분장비과민성효천환자분위2조,A 조58례(ACT 평분≥20),B 조77례(ACT 평분<20),병검측기기도반응성。채용 Pearson 상관검험분석 ACT 평분여 hs-CRP 급기도반응성적상관성。결과비과민성효천환자담액중 hs-CRP 함량명현고우대조조,이폐공능각지표 FEV1、FEV1%、PEF 급 PEF%칙현저저우대조조(균 P <0.01)。비과민성효천 B 조환자담액중 hs-CRP 수평교 A 조고(P <0.01),이반영기도반응성적 FEV1개변량균현저저우 A 조(P <0.05)。ACT 평분여 hs-CRP 함량정부상관(r =-0.76,P <0.01),여 FEV1증가정도(%)화절대치증가량(mL)정정상관(r=0.85,P <0.05;r =0.916,P <0.05)。결론ACT 평분여비과민성효천환자승고적 hs-CRP 수평급기도반응성밀절상관,정학사용해평분유조우간편、쾌첩지진행병정평고。
ABSTRACT:Objective To investigate the correlations of asthma control test(ACT)scores with sputum high-sensitivity C-reactive protein(hs-CRP)levels and airway reactivity in patients with nonallergic asthma.Methods The hs-CRP contents in sputum and pulmonary function were measured in 135 patients with nonallergic asthma(nonallergic asthma group)and 50 healthy sub-jects(control group).According to the ACT score,patients with nonallergic asthma were further divided into two groups:group A(ACT score≥20,n=58)and group B(ACT score<20,n=77). The airway reactivity was determined in both groups.The correlations of ACT scores with spu-tum hs-CRP contents and airway reactivity were analyzed by Pearson correlation analysis.Results Compared with control group,the hs-CRP contents in sputum increased and the FEV1,FEV1%, PEF and PEF% decreased in nonallergic asthma group(P <0.01).Compared with group A,the hs-CRP contents in sputum increased and the change in FEV1 decreased in group B(P <0.01 or P <0.05).The ACT scores were negatively correlated with the hs-CRP levels(r = -0.76,P <0.01),but positively correlated with the increased degree(%)and amount(mL)of FEV1 (r =0.85,P <0.05 and r =0.916,P <0.05,respectively ).Conclusion The ACT scores are closely correlated with sputum hs-CRP levels and airway reactivity in patients with nonallergic asthma. Therefore,ACT scores can be used as a simple measure to assess the development of nonallergic asthma.
