目的：观察左乙拉西坦（levetiracetam，Lev）治疗癫痫患儿的临床疗效和安全性。方法：对2010年1月至2013年6月我院收治的78例小儿癫痫患者进行Lev单药治疗，采用自身对照随访研究。Lev起始剂量10mg/kg·d，一天2次口服，每周增加5~10mg/kg·d，目标剂量为30~40mg/kg·d，维持剂量35mg/kg·d。门诊复查及电话随访，观察其发作频率和不良反应。结果：78例患儿中，完全控制发作59例，良好3例，有效8例，总有效率89.7%。治疗前后发作频率差异有统计学意义（P＜0.05）。不良反应以头痛、嗜睡、厌食、情绪激动、行为异常为主，对症治疗或达到维持剂量后自行消失。结论：Lev作为一种新型抗癫痫药物，单药治疗小儿癫痫作用广，有良好疗效及安全性，值得在临床治疗中推广和应用。
목적：관찰좌을랍서탄（levetiracetam，Lev）치료전간환인적림상료효화안전성。방법：대2010년1월지2013년6월아원수치적78례소인전간환자진행Lev단약치료，채용자신대조수방연구。Lev기시제량10mg/kg·d，일천2차구복，매주증가5~10mg/kg·d，목표제량위30~40mg/kg·d，유지제량35mg/kg·d。문진복사급전화수방，관찰기발작빈솔화불량반응。결과：78례환인중，완전공제발작59례，량호3례，유효8례，총유효솔89.7%。치료전후발작빈솔차이유통계학의의（P＜0.05）。불량반응이두통、기수、염식、정서격동、행위이상위주，대증치료혹체도유지제량후자행소실。결론：Lev작위일충신형항전간약물，단약치료소인전간작용엄，유량호료효급안전성，치득재림상치료중추엄화응용。
Objective:To evaluate the clinical efficacy and safety of levetiracetam monotherapy in children with epilepsy. Methods:78 children patients with epilepsy who were diagnosed in our hospital from January 2010 and June 2013, were treated by Levetiracetam, then follow-up visited. Dose of LEV was initially 10mg/kg·d, twice a day, every week the dose increased 5~10mg/kg·d, maintained at 30~40mg/kg·d, and finally was increased to 35mg/kg·d. Followed up to observe the seizure frequency and adverse reactions. Results:Among the 78 cases, complete seizure control was found in 59 cases, excellence in 3 cases, effective in 8 cases, the total effective rate was 89.7%.There was significant difference of seizure frequency before and after therapy(P<0.05).The major side effects observed were headache, lethargy, anxiety, abnormal behavior etc,and the symptoms were disappeared within the treatment. Conclusions:LEV as a new type of antiepileptic drugs, is efficacious of children with epilepsy, and has few adverse reactions and should be wildly applied to epileptic children.
