目的探讨支撑喉镜下显微微瓣缝合技术在声带增生性疾病手术治疗中对嗓音的改善效果。方法对78例声带病变(声带囊肿25例,任克层水肿17例,声带广基大息肉33例,声带原位癌3例),采用显微镜下切除病变组织并采用微瓣技术修复,并缝合创面。术前和术后1周、2周分别进行频闪喉镜检查及嗓音声学参数检测。结果78例均一次切除病变,未发现残留,无并发症发生。术后1周声嘶好转和声带黏膜波恢复正常的例数为69例,术后2周为77例。术后1周和2周嗓音参数检测:基频(F0)、基频微扰(jitter)、振幅微扰(shimmer)均有较明显改善,与术前各声学参数值相比差异有显著性( P <0.05)。结论显微微瓣缝合技术在声带增生性疾病手术治疗中,既可彻底切除病变,又能很好地保留黏膜的完整性,缩短恢复时间,减小术后声带瘢痕的形成,提高发音品质。

목적탐토지탱후경하현미미판봉합기술재성대증생성질병수술치료중대상음적개선효과。방법대78례성대병변(성대낭종25례,임극층수종17례,성대엄기대식육33례,성대원위암3례),채용현미경하절제병변조직병채용미판기술수복,병봉합창면。술전화술후1주、2주분별진행빈섬후경검사급상음성학삼수검측。결과78례균일차절제병변,미발현잔류,무병발증발생。술후1주성시호전화성대점막파회복정상적례수위69례,술후2주위77례。술후1주화2주상음삼수검측:기빈(F0)、기빈미우(jitter)、진폭미우(shimmer)균유교명현개선,여술전각성학삼수치상비차이유현저성( P <0.05)。결론현미미판봉합기술재성대증생성질병수술치료중,기가철저절제병변,우능흔호지보류점막적완정성,축단회복시간,감소술후성대반흔적형성,제고발음품질。
Objective To investigate the improvement of the voice effect in proliferative disorder of vocal cords by using micro flap suture technique under laryngoscope. Methods 78 cases of vocal cords benign disorder(25 cases of vocal cords cyst,17 cases of Reinke′s edema,33 cases of broad - based vocal cords polyp,3 cases of carcinoma in situ). The pathologic tissues were removed by micro flay technique,and sutured by the mucosal flap technique,and measured by stroboscopy and voice acoustic analysis in pre - operation and post - operation in 1 month and 2 months. Results The pathologic tissues of 78 cases were removed completely with no complication. The number of cases that hoarseness improved and the mucosa wave of vocal cords back to normal are 69 cases and 77 cases,respectively after operation in 1 week and 2 weeks. Parameters of voice analysis after operation in 1 week and 2 weeks:the fundamental frequency(F0),jitter,shimmer have a significant improvement. There is a significant difference in different parameters of voice. Conclusion The micro - suture technology is effective in improving the voice quality. The micro flap suture technique can completely remove the pathological tissue,and retain the integrity of the mucosa,shorten the recovery time,reduce scar formation of vocal cords and improve the quality of the voice.
