目的：探讨视神经脊髓炎（ neuromyelitis optica， NMO）的发病特点、临床表现和必要的实验室检查，以提高早期诊断的正确率。方法回顾性分析武警总医院神经内科72例NMO患者的发病特点、临床表现、病程进展和脑脊液等相关检查，并进行总结分析。结果（1）平均发病年龄（36．2±10．3）岁，复发-缓解型病程66例，以视神经炎为首发症状40例；（2）血清AQP4抗体的阳性率为90％，脑脊液AQP4抗体的阳性率为69．4％，30％NMO患者自身抗体谱异常，19．4％患者的脑脊液电泳可见寡克隆区带；（3）脊髓核磁共振成像（ magnetic resonance imaging，MRI）中胸髓、颈髓、腰髓受累率分别为84．7％、69．4％和19．4％。结论 NMO以中年女性发病为主，病程多为缓解-复发型，血清和脑脊液中AQP4抗体多为阳性。
목적：탐토시신경척수염（ neuromyelitis optica， NMO）적발병특점、림상표현화필요적실험실검사，이제고조기진단적정학솔。방법회고성분석무경총의원신경내과72례NMO환자적발병특점、림상표현、병정진전화뇌척액등상관검사，병진행총결분석。결과（1）평균발병년령（36．2±10．3）세，복발-완해형병정66례，이시신경염위수발증상40례；（2）혈청AQP4항체적양성솔위90％，뇌척액AQP4항체적양성솔위69．4％，30％NMO환자자신항체보이상，19．4％환자적뇌척액전영가견과극륭구대；（3）척수핵자공진성상（ magnetic resonance imaging，MRI）중흉수、경수、요수수루솔분별위84．7％、69．4％화19．4％。결론 NMO이중년녀성발병위주，병정다위완해-복발형，혈청화뇌척액중AQP4항체다위양성。
Objective To explore clinical features, clinical manifestations and necessary laboratory tests of neuromyelitis optica (NMO), in an attempt of improving the accuracy of early diagnosis.Methods 72 cases from the General Hospital of Chinese People’s Armed Forces were collected.Their epidemiological features, clinical manifestations and the necessary laboratory tests were then ana-lyzed.Results (1) The average morbidity age was 36.2 ±10.3 year-old.There were 66 cases with relapse-release course of disease.40 cases NMO patients were with optic neuritis as the first symptom.(2) The positive rate of AQP4 antibody in serum is as high as 90.3%, whereas it comes to 69.4% in cerebrospinal fluid.30% of patients had abnormal autoantibody and oligodonal bands can be seen in 19.4%of patients’ cerebrospinal fluid.(3) Spinal cord magnetic resonance imaging (MRI) showed that the disease involved rate in thoracic spinal cord, cervical cord, and lumbar spinal cord were 84.7%, 69.4%and 19.4%, respectively.Conclusions NMO occurs mostly in middle-aged woman, and the common course of disease is relapse-release.The AQP4 antibodies are mostly positive both in ce-rebrospinal fluid and serum.
