目的:探讨原发性肾病综合征(PNS)与血小板活化因子水解酶(PAF-AH)的相关性。方法选择湖北省黄石市中心医院2013年1—6月收治的 PNS 患者48例,按病理类型分为单纯性肾病综合征(STNS)组31例及肾炎型肾病综合征(NTNS)组17例,并给予6个月激素治疗后根据对激素治疗效应差异分为激素敏感型肾病综合征(SSNS)组19例,激素耐药型肾病综合征(SRNS)组15例,激素依赖型肾病综合征(SDNS)组14例;选择同期健康体检人员30例作为对照组。比较各组间 PAF-AH 活性,分析其与不同类型 PNS 的相关性。结果 PAF-AH 活性 STNS 组均高于 NTNS 组及对照组,且 NTNS 组高于对照组(P <0.05);SSNS 组均高于 SRNS 组、SDNS 组、对照组,且 SRNS 组、SDNS 组均高于对照组( P <0.05)。 PAF-AH 活性与 STNS、NTNS、SSNS、SRNS、SDNS 呈正相关( r =0.618、0.524、0.717、0.567、0.327, P <0.05)。结论 PNS 患者 PAF-AH 活性显著高于正常人群,且不同类型 PNS 患者 PAF-AH 活性不同,可能是该病对激素治疗反应不一致的原因。

목적:탐토원발성신병종합정(PNS)여혈소판활화인자수해매(PAF-AH)적상관성。방법선택호북성황석시중심의원2013년1—6월수치적 PNS 환자48례,안병리류형분위단순성신병종합정(STNS)조31례급신염형신병종합정(NTNS)조17례,병급여6개월격소치료후근거대격소치료효응차이분위격소민감형신병종합정(SSNS)조19례,격소내약형신병종합정(SRNS)조15례,격소의뢰형신병종합정(SDNS)조14례;선택동기건강체검인원30례작위대조조。비교각조간 PAF-AH 활성,분석기여불동류형 PNS 적상관성。결과 PAF-AH 활성 STNS 조균고우 NTNS 조급대조조,차 NTNS 조고우대조조(P <0.05);SSNS 조균고우 SRNS 조、SDNS 조、대조조,차 SRNS 조、SDNS 조균고우대조조( P <0.05)。 PAF-AH 활성여 STNS、NTNS、SSNS、SRNS、SDNS 정정상관( r =0.618、0.524、0.717、0.567、0.327, P <0.05)。결론 PNS 환자 PAF-AH 활성현저고우정상인군,차불동류형 PNS 환자 PAF-AH 활성불동,가능시해병대격소치료반응불일치적원인。
Objective To investigate the correlation of primary nephrotic syndrome (PNS) and platelet activa-ting factor acetylhydrolase (PAF-AH). Methods A total of 48 PNS patients from January to June 2013 were divided in-to simple type nephrotic syndrome group (STNS group, n = 31) and nephritic type nephrotic syndrome group (NTNS group, n = 17) according to pathological types. After hormonal therapy for 6 months, the 48 patients were divided into steroid-sensitive nephritic syndrome group ( SSNS group, n = 19), steroid-resistant nephritic syndrome group ( SRNS group, n = 15) and steroid-dependent nephritic syndrome group (SDNS group, n = 14) according to the effect differences in hormonal therapy, and then 30 healthy persons taking medical examination at the same period were selected as a con-trol group. The PAF-AH activity in each group was determined, and then its correlations with different types of PNS were analyzed. Results The value of PAF-AH activity in STNS group was significantly higher than that in NTNS group, and the activity value in NTNS group was significantly higher than that in control group (P < 0. 05); the activity value in SSNS group was significantly higher than those in SRNS, SDNS and control groups, and the activity values in SRNS and SDNS groups were significantly higher than that in control group (P < 0. 05). The activity of PAF-AH was positively cor-related in STNS, NTNS, SSNS, SRNS and SDNS groups (r = 0. 618, 0. 524, 0. 717, 0. 567, 0. 327, P < 0. 05). Con-clusion The values of PAF-AH activity in PNS patients are significantly higher than those in normal people, and the dif-ferent types of PNS patients have different PAF-AH activities, which may explain why there are different responses to hor-monal therapy.
