目的：观察复方大承气汤治疗重症急性胰腺炎（ SAP ）的疗效及对血清氧自由基水平的的影响。方法将65例SAP患者随机分为治疗组和对照组。对照组采用禁食水、持续胃肠减压、抑酶、维持水电解质平衡治疗。治疗组在对照组基础上加用复方大承气汤，监测治疗前及治疗后10 d 2组血清中超氧化物歧化酶（ SOD）、丙二醛（ MDA）、血清淀粉酶及脂肪酶变化情况，统计症状缓解时间及临床疗效。结果治疗后治疗组临床症状缓解时间较对照组明显缩短（P＜0．01），2组SOD水平较治疗前明显上升（P均＜0．01），血清淀粉酶、脂肪酶、MAD较治疗前明显下降（P均＜0．01），且治疗组较对照组变化更明显（P均＜0．01）。治疗组总有效率高于对照组（P＜0．05）。结论复方大承气汤能有效清除SAP患者血清氧自由基，对SAP具有良好的治疗作用。
목적：관찰복방대승기탕치료중증급성이선염（ SAP ）적료효급대혈청양자유기수평적적영향。방법장65례SAP환자수궤분위치료조화대조조。대조조채용금식수、지속위장감압、억매、유지수전해질평형치료。치료조재대조조기출상가용복방대승기탕，감측치료전급치료후10 d 2조혈청중초양화물기화매（ SOD）、병이철（ MDA）、혈청정분매급지방매변화정황，통계증상완해시간급림상료효。결과치료후치료조림상증상완해시간교대조조명현축단（P＜0．01），2조SOD수평교치료전명현상승（P균＜0．01），혈청정분매、지방매、MAD교치료전명현하강（P균＜0．01），차치료조교대조조변화경명현（P균＜0．01）。치료조총유효솔고우대조조（P＜0．05）。결론복방대승기탕능유효청제SAP환자혈청양자유기，대SAP구유량호적치료작용。
Objective It is to observe the curative effect of compound Dachengqi decoction on severe acute pancreatitis ( SAP) and the effect on the levels of serum oxygen -free radicals.Methods 65 cases of patients with SAP were randomly di-vided into treatment group and control group .The control group was treated with diet resistance , continue gastrointestinal de-compression , inhibition of enzyme and keeping waterelectrolyte balance , and the treatment group was treated with compound Dachengqi decoction on the basis of treatment of the control group .The changes of serum SOD , MDA, amylase and lipase were determined in both groups before treatment and 10 d after treatment , reliving time of symptoms and clinical curative effect were observed and compared .ResultsThe reliving time of symptoms of treatment group after treatment was significantly shorter than that of control group (P<0.01), the levels of serum SOD were higher, and the levels of MDA, amylase and lipase were lower than those before treatment in both groups (P<0.01), and the improvements in treatment group were more significant than that in control group (P<0.01).The total effective rate of treatment group was higher than that of control group (P<0. 05).Conclusion Compound Dachengqi decoction can effectively clear serum oxygen -free radicals in the patients with SAP , and it has a good curative effect on this disease .
