目的：观察复方黄柏液创面灌洗对肛周坏死性筋膜炎术后创面愈合的影响。方法将52例肛门周围继发肛门会阴部感染所导致的坏死性筋膜炎患者随机分为对照组和治疗组，每组26例。2组手术方法相同，治疗组术后采用复方黄柏液灌洗创面，对照组采用常规方法处理创面。观察2组术后1～15 d每晚疼痛视觉模拟评分（ VAS），统计2组水肿改善总有效率、术后发热发生率和切口愈合时间。结果术后7～15 d治疗组VAS评分总体低于对照组（ P均＜0．05）。治疗组和对照组水肿改善总有效率分别为92％和73％，术后发热发生率分别为0％和23％，切口愈合时间分别为（32．7±4．0）d和（52．8±6．2）d，2组比较差异均有统计学意义（P均＜0．05）。结论复方黄柏液创面灌洗对在缓解术后疼痛、减轻水肿、抗感染和促进愈合创面效果明显优于常规治疗，是一种较为理想的方法。
목적：관찰복방황백액창면관세대항주배사성근막염술후창면유합적영향。방법장52례항문주위계발항문회음부감염소도치적배사성근막염환자수궤분위대조조화치료조，매조26례。2조수술방법상동，치료조술후채용복방황백액관세창면，대조조채용상규방법처리창면。관찰2조술후1～15 d매만동통시각모의평분（ VAS），통계2조수종개선총유효솔、술후발열발생솔화절구유합시간。결과술후7～15 d치료조VAS평분총체저우대조조（ P균＜0．05）。치료조화대조조수종개선총유효솔분별위92％화73％，술후발열발생솔분별위0％화23％，절구유합시간분별위（32．7±4．0）d화（52．8±6．2）d，2조비교차이균유통계학의의（P균＜0．05）。결론복방황백액창면관세대재완해술후동통、감경수종、항감염화촉진유합창면효과명현우우상규치료，시일충교위이상적방법。
Objective It is to investigate the clinical efficacy of compound Huangbai liquid lavage incision therapy for wound healing after surgery for perianal necrotizing fasciitis .Methods 52 patients with perianal necrotizing fasciitis were ran-domly assigned to treatment group (n=26) and control group (n=26), both the groups were treated with the same surgery , the wound surface of the patients in treatment group was lavaged with compound Huangbai liquid while the surface was lavaged with normal method in control group .The VAS scores of pain at night from 1 to 15 days after treatment , Subsidence of hydro-sarca, incidence of postoperative infection and healing time of incision were observed and compared in both groups .Results The pain scores at night of the 7-15 days after operation in the treatment group were lower than those in the control group ( P<0.05 ) .The incidences of hydrosarca subsidence of the treatment group and control group were respectively 92% and 73%, while the incidences of postoperative infection of the treatment group and the control group were respectively 0%and 23%, the healing time of incision were (32.7 ±4.0)d and (52.8 ±6.2)d respectively, the differences were all statistical significant (P<0.05).Conclusion The compound Huangbai liquid lavage incision is a perfect therapy in the treatment for perianal necro -tizing fasciitis, whose effects are better than normal therapy in reliving postoperative pain , edema, inhibiting infection and pro-moting wound healing .
