脓毒症是儿童的危重症之一,有很高的病死率和致残率.这类患者的手术是麻醉医师面临的重要挑战之一.报道1名脓毒症合并多浆膜腔积液的学龄儿童在全身麻醉下行心包和胸膜腔引流术的麻醉经过.应用“儿童脓毒症国际共识”进行术前评估,根据患儿病情制定个体化的麻醉方案,围术期积极应用超声心动图监测等手段是保障患者安全的重要措施.

Pediatric sepsis is a severe life-threatening disease with high mortality and mobility.Anesthesia management of pediatric patients with sepsis raises challenge to anesthesiologists.Here we report a case of anesthesia management of a school-age child with sepsis and polyserositis.According to international consensus conference on pediatric sepsis criteria for pediatric systemic inflammatory response syndrome and sepsis diagnosis,a personalized anesthetic plan was made and successfully performed.New invasive monitor tools such as transthoracic echocardiography and tranesophageal echocardiogrhy plays vital roles in the anesthesia management of the case.
