目的：观察输尿管镜下行气压弹道碎石术治疗输尿管结石的临床疗效。方法：回顾性分析行输尿管镜下气压弹道碎石术（USRL）518例患者，共存在568侧输尿管结石，统计所有患者治疗成功率并研究预防策略。结果：518例患者共568侧输尿管结石，治疗成功503侧（88.56％），输尿管镜手术失败改行开放取石18例（3.47％），单纯置管引流47例（9.07％）；术后并发症发生40例（7.72％），其中上段输尿管结石成功111例（75.00％），中段成功175例（90.20％），下段成功例217例（96.02％），上段输尿管结石治疗成功率与中下段比较差异具有统计学意义（P ＜0.05）。结论：USRL 治疗输尿管结石具有较高的临床疗效，可作为输尿管结石的临床首选疗法，但对于输尿管上、中、下段结石治疗效果存在差异，在行输尿管上段结石手术治疗时应谨慎选择该方案。
목적：관찰수뇨관경하행기압탄도쇄석술치료수뇨관결석적림상료효。방법：회고성분석행수뇨관경하기압탄도쇄석술（USRL）518례환자，공존재568측수뇨관결석，통계소유환자치료성공솔병연구예방책략。결과：518례환자공568측수뇨관결석，치료성공503측（88.56％），수뇨관경수술실패개행개방취석18례（3.47％），단순치관인류47례（9.07％）；술후병발증발생40례（7.72％），기중상단수뇨관결석성공111례（75.00％），중단성공175례（90.20％），하단성공례217례（96.02％），상단수뇨관결석치료성공솔여중하단비교차이구유통계학의의（P ＜0.05）。결론：USRL 치료수뇨관결석구유교고적림상료효，가작위수뇨관결석적림상수선요법，단대우수뇨관상、중、하단결석치료효과존재차이，재행수뇨관상단결석수술치료시응근신선택해방안。
Objective To investigate the clinical effect of transureteroscopic pneumatic lithotripsy on treating ureteral calculi. Methods Retrospective analyzed 518 ureteroscopic pneumatic lithotripsy (USRL)patients ,there were 568 sides of ureteral stones in all patients,counted treatment success rate and studied prevention strategies.Results There were 568 ureteral stones in 518 cases of u-reteral calculi patients,the success case was 503ureteral (88.56%),18 cases underwent open ureterolithotomy failure used ureteros-copy operation (3.47%),47 cases simple drainage (9.07%);40 cases complications (7.72%).There were 111 success cases (75.00%)of epimere ureteral calculi,175 success cases (90.20%)of middle ureteral calculi,217 success cases (96.02%)of lower ureteral calculi,comparison between two groups'data had the remarkable difference,P <0.05,with statistical significance. Conclusion The clinical efficacy of USRL in treating ureteral calculi is higher,it can be used as the preferred clinical therapy of ureter-al calculi,but there is difference between upper ,middle and lower ureter calculi treatment effect,we should be cautious when choo-sing operation scheme of upper ureteral calculi operation .
