目的:研究红花注射液在慢性肺源性心脏病急性加重过程中凝血系统、血气分析及肺动脉压力的影响。方法慢性肺源性心脏病病人60名,随机分为2组,每组30人,匹配年龄,性别等因素,对照组给予常规方法治疗,红花注射液组在常规治疗的基础上加用红花注射液20 ml,10天为一疗程。治疗前后,检测血气分析,内皮素-1(ET-1)、VIIIR:Ag、D-二聚体,平均肺动脉压(mPAP)。结果与对照组相比,红花注射液组血浆ET-1、VIIIR:Ag、D-二聚体水平在治疗前无明显差异(P>0.05),治疗后均有明显下降,差异有统计学意义(P<0.05);治疗后,与对照组相比,红花注射液组PO2、SaO2明显升高,mPAP明显降低,两组比较,差异有统计学意义( P<0.05)。结论红花注射液能够显著降低患者血浆VIIIR:Ag、D-二聚体水平,改善血液血栓前状态,预防血栓形成;能降低血浆ET水平,降低肺动脉高压,改善肺通气,纠正缺氧状态。

목적:연구홍화주사액재만성폐원성심장병급성가중과정중응혈계통、혈기분석급폐동맥압력적영향。방법만성폐원성심장병병인60명,수궤분위2조,매조30인,필배년령,성별등인소,대조조급여상규방법치료,홍화주사액조재상규치료적기출상가용홍화주사액20 ml,10천위일료정。치료전후,검측혈기분석,내피소-1(ET-1)、VIIIR:Ag、D-이취체,평균폐동맥압(mPAP)。결과여대조조상비,홍화주사액조혈장ET-1、VIIIR:Ag、D-이취체수평재치료전무명현차이(P>0.05),치료후균유명현하강,차이유통계학의의(P<0.05);치료후,여대조조상비,홍화주사액조PO2、SaO2명현승고,mPAP명현강저,량조비교,차이유통계학의의( P<0.05)。결론홍화주사액능구현저강저환자혈장VIIIR:Ag、D-이취체수평,개선혈액혈전전상태,예방혈전형성;능강저혈장ET수평,강저폐동맥고압,개선폐통기,규정결양상태。
Objective:To study the influence of Carthamus tinctorius on coagulation system,blood gas analysis and pul-monary arterial pressure in exacerbation of chronic pulmonary heart disease. Methods:Forty patients with exacerbation of chronic pulmonary heart disease were randomly divided into two groups:the control group and the Carthamus tinctorius group,with 20 patients in each group. The control group were given conventional treatment and the Carthamus tinctorius group were given both conventional treatment and Carthamus tinctorius,20ml per day for 10 days in one period of treat-ment. Before and after treatment,ET-1,VIIIR:Ag,mPAP,D-dimer and blood gas analysis were detectded. Results:Compared to the control group,after treatment,the Carthamus tinctorius group has lower ET-1,VIIIR:Ag,D-dimer,mPAP and higher PO2 and SaO2 . Conclusion:Carthamus tinctorius can significantly reduce the plasma VIIIR:Ag and D- dimer level,prevent thrombosis,decrease the plasma ET levels,reduce pulmonary arterial hypertension,improve pulmonary ven-tilation and correct hypoxia.
