目的:探讨激光打孔法辅助孵化对复苏周期中囊胚移植妊娠结局的影响。方法收集复苏周期中行囊胚移植患者的568个周期,其中352个周期于冻存囊胚复苏后,行激光打孔辅助孵化后移植(AH 组),216个复苏周期囊胚移植未行激光打孔辅助孵化(非 AH 组);分析两组的生化妊娠率、临床妊娠率、种植率、流产率、继续妊娠率、多胎妊娠率及单卵双胎率。结果两组患者的年龄、不孕原因、排卵日内膜厚度、平均移植胚胎数组间差异无统计学意义。AH 组生化妊娠率、临床妊娠率、种植率、继续妊娠率显著高于非 AH组(P <0.05),流产率显著低于非 AH 组(P <0.05),多胎妊娠率及单卵双胎率略高于非 AH 组,但差异无统计学意义。结论复苏周期囊胚移植前采用激光打孔辅助孵化可以提高妊娠率,降低流产率。

목적:탐토격광타공법보조부화대복소주기중낭배이식임신결국적영향。방법수집복소주기중행낭배이식환자적568개주기,기중352개주기우동존낭배복소후,행격광타공보조부화후이식(AH 조),216개복소주기낭배이식미행격광타공보조부화(비 AH 조);분석량조적생화임신솔、림상임신솔、충식솔、유산솔、계속임신솔、다태임신솔급단란쌍태솔。결과량조환자적년령、불잉원인、배란일내막후도、평균이식배태수조간차이무통계학의의。AH 조생화임신솔、림상임신솔、충식솔、계속임신솔현저고우비 AH조(P <0.05),유산솔현저저우비 AH 조(P <0.05),다태임신솔급단란쌍태솔략고우비 AH 조,단차이무통계학의의。결론복소주기낭배이식전채용격광타공보조부화가이제고임신솔,강저유산솔。
Objective To explore the effects of artificial drilling by laser-assisted hatching(AH)on the outcomes of frozen-thawed blastocyst transfer. Methods 568 frozen-thawed blastocyst transfer cycles were selected . 352 cy-cles underwent transfer after drilling by laser-assisted hatching(AH group),216 cycles transfer without laser-assis-ted hatching(non-AH group). The rates of biochemical pregnancy,clinical pregnancy,implantation,abortion, ongoing pregnancy,multiple pregnancy and monozygotic twins were statistically analysed in the two groups. Results There was no difference in the patients age,the cause of infertility,endometrium thickness on ovulation day,and average numbers of the embryos transferred. Biochemical pregnancy rate,the clinical pregnancy rate,implantation rate,ongoing pregnancy rate in AH group were significantly higher than those in non-AH group(P < 0. 05). The abortion rate was significantly lower than in non-AH group( P < 0. 05),and the multiple pregnancy rate and monozygotic twinning rate were slightly higher than in non-AH group,without significant difference. Conclusion Drilling by laser-assisted hatching before frozen-thawed blastocyst transfer can increase the rates of implantation and pregnancy,while reduce the abortion rate.
