目的:探讨老年支气管哮喘患者呼出气一氧化氮( FeNO)水平与肺功能的相关性。方法对68例老年哮喘患者给予吸入舒利迭和异丙托溴铵喷雾剂,疗程为1个月。比较50例健康体检者及哮喘患者治疗前、治疗后FeNO、肺功能变化,采用哮喘控制测试( ACT)评分评价其疗效。结果与对照组比较,哮喘组治疗前FeNO、Eos、N水平明显升高,各肺功能指标明显下降( P<0.05);治疗后,各指标均明显改善( P<0.05),PEF、FEV1、Eos、N基本恢复至对照组水平(P>0.05);治疗后完全控制组、部分控制组各项指标均优于未控制组,完全控制组FeNO、Eos、N水平均明显低于部分控制组( P<0.05);治疗前,哮喘组患者FeNO水平与Eos、N呈正相关(P<0.05),与肺功能指标呈负相关(P<0.05),但治疗后,FeNO水平与Eos、N呈正相关(P<0.05)。结论 FeNO水平在哮喘患者中明显升高,结合肺功能检查,有助于提高老年哮喘的诊断及治疗水平。

목적:탐토노년지기관효천환자호출기일양화담( FeNO)수평여폐공능적상관성。방법대68례노년효천환자급여흡입서리질화이병탁추안분무제,료정위1개월。비교50례건강체검자급효천환자치료전、치료후FeNO、폐공능변화,채용효천공제측시( ACT)평분평개기료효。결과여대조조비교,효천조치료전FeNO、Eos、N수평명현승고,각폐공능지표명현하강( P<0.05);치료후,각지표균명현개선( P<0.05),PEF、FEV1、Eos、N기본회복지대조조수평(P>0.05);치료후완전공제조、부분공제조각항지표균우우미공제조,완전공제조FeNO、Eos、N수평균명현저우부분공제조( P<0.05);치료전,효천조환자FeNO수평여Eos、N정정상관(P<0.05),여폐공능지표정부상관(P<0.05),단치료후,FeNO수평여Eos、N정정상관(P<0.05)。결론 FeNO수평재효천환자중명현승고,결합폐공능검사,유조우제고노년효천적진단급치료수평。
Objective To explore the correlation between exhaled nitric oxide ( FeNO) and pulmonary func-tion in elderly patients with bronchial asthma. Methods 68 elderly asthma patients were given seretide and iprat-ropium bromide for 1 month. FeNO and pulmonary function were detected for elderly asthma before and after treat-ment and compared with 50 healthy controls. Results Compared with the control group, the levels of FeNO, Eos and N were higher in the asthma group before the treatment, and their lung function decreased obviously (P<0. 05). After the treatment, all the indicators improved more significantly than those before treatment ( P<0. 05 ) , and the levels of PEF, FEV1, Eos and N were back to normal value (P>0. 05). The indicators in the full control group and the partly control group were superior to the control group after the treatment. The levels of FeNO, Eos and N in the full control group were significantly lower than those in the partly control group (P<0. 05). Before the treatment, the level of FeNO in asthma patients was positively correlated with the levels of Eos and N ( P<0. 05 ) , and negatively correlated with lung function indicators ( P<0. 05 ) . After the treatment, FeNO was positively correlated with Eos and N (P<0. 05). Conclusion The level of FeNO increases significantly in patients with asthma, and it helps to improve the diagnosis and treatment level of elderly asthma while combining with lung function.
