目的：探讨浸润性乳腺Paget病(invasive mammary Paget disease，invMPD)的临床病理特征。方法：回顾性分析3例浸润性乳腺Paget病的临床资料和病理资料并复习文献。结果：3例患者均有乳头糜烂，1例伴有乳头溃疡，1例同侧乳腺左侧外上象限浸润性癌，1例伴有同侧乳腺高级别导管内癌。组织学表现为具有典型的乳腺Paget病的特征，可见小的浸润灶突破基底膜，浸润真皮浅层。免疫组织化学示肿瘤细胞均表达ER、CK7、EMA；2例表达PR，2例HER-2(3+)，1例HER-2(2+)，Ki67从40%~60%；S-100、P63均阴性。结论：浸润性乳腺Paget病是一种罕见的肿瘤，与非浸润性乳腺Paget病(non-invasive mammary Paget disease，non-invMPD)有相似的预后。
목적：탐토침윤성유선Paget병(invasive mammary Paget disease，invMPD)적림상병리특정。방법：회고성분석3례침윤성유선Paget병적림상자료화병리자료병복습문헌。결과：3례환자균유유두미란，1례반유유두궤양，1례동측유선좌측외상상한침윤성암，1례반유동측유선고급별도관내암。조직학표현위구유전형적유선Paget병적특정，가견소적침윤조돌파기저막，침윤진피천층。면역조직화학시종류세포균표체ER、CK7、EMA；2례표체PR，2례HER-2(3+)，1례HER-2(2+)，Ki67종40%~60%；S-100、P63균음성。결론：침윤성유선Paget병시일충한견적종류，여비침윤성유선Paget병(non-invasive mammary Paget disease，non-invMPD)유상사적예후。
Objective:To study the clinicopathologic characteristics of invasive mammary Paget disease. Methods:Retrospective analysis of the clinicopathologic data of 3 cases of invasive mammary Paget disease and review literature. Results:A total of 3 cases displayed papillary erosion, 1 case with ulcer, 1 case with invasive carcinoma and 1 case with DCIS. Microscopically, 3 cases displayed typical characteristics of Paget disease, accompanied dermal invasion. Immunohistochemical study showed CK7, EMA, ER were positive in 2 cases, HER-2, PR was positive in 2 case and S-100, P63 were negative in all. Conclusion:Invasive mammary Paget disease is a rare neoplasm and the similar prognosis with non-invasive mammary Paget disease.
