目的：总结社区全科医生开展家庭病床的服务模式。方法：全科医生、护士宣传家庭病床服务的意义，根据家床患者健康状况，制定合理的个体化医疗方案，提供预防、治疗、保健、康复、健康教育等一体化的综合服务。结果：采用的个体化医疗方案能改善患者健康状况，提高生活质量，且降低医疗费用，家床期间未发生医疗纠纷事故。结论：制定合理的个体化医疗方案是顺应特殊人群对健康需求的新型医疗服务模式，对提高人群生活质量、促进医患和谐、减轻患者医疗负担有着积极的意义。

목적：총결사구전과의생개전가정병상적복무모식。방법：전과의생、호사선전가정병상복무적의의，근거가상환자건강상황，제정합리적개체화의료방안，제공예방、치료、보건、강복、건강교육등일체화적종합복무。결과：채용적개체화의료방안능개선환자건강상황，제고생활질량，차강저의료비용，가상기간미발생의료규분사고。결론：제정합리적개체화의료방안시순응특수인군대건강수구적신형의료복무모식，대제고인군생활질량、촉진의환화해、감경환자의료부담유착적겁적의의。
Objective:To summarize the service mode of hospital at home carried out by general practitioners in community. Methods:General practitioners and nurses publicized the significance of hospital at home and formulate reasonable individualized medical scheme according to the health situation of patients with hospital at home and provided integrated services including prevention,treatment,health care,rehabilitation,health education and so on.Results:Individualized health care could improve the patients health and their quality of life,and reduce medical costs.There was no dispute of medical accident during hospital at home. Conclusion:Reasonable individual medical plan formulated was new type of medical service mode complying with the health needs of special people,which was positive significance to improve the population quality of life,promoting harmonious doctor-patient,reducing the patients' medical burden.
