目的：观察健脾泄浊法治疗急性期痛风性关节炎的临床疗效。方法将104例急性期痛风性关节炎患者随机分为两组。对照组52例，西药常规处理，治疗组52例，在西药常规处理基础上加用健脾泄浊法组方方药加减口服治疗，疗程为14 d。观察症状改善情况、炎性指标、尿 pH 值、血尿酸（ BUA）和临床安全性等指标变化情况。结果治疗组总有效率为92.31%，对照组为88.46%，两组疗效比较，差异有统计学性意义（P ﹤0.05）；治疗后两组患者的关节肿痛指数、VAS 评分、红细胞沉降率（ESR）、白细胞（WBC）、超敏 C 反应蛋白（hs - CRP）、中性粒细胞百分比（N%）、pH值等炎症指标均明显下降（P ﹤0.05）；两组疼痛时间比较差异有统计学意义（P ﹤0.01）；与对照组比较，治疗组无胃肠道不良反应（P ﹤0.01）、肝肾损害等明显不良反应。结论健脾泄浊方治疗急性期痛风性关节炎能够有效改善临床症状、降低血尿酸、碱化尿液、缩短疗程，且临床安全性优于单纯西药治疗者。
목적：관찰건비설탁법치료급성기통풍성관절염적림상료효。방법장104례급성기통풍성관절염환자수궤분위량조。대조조52례，서약상규처리，치료조52례，재서약상규처리기출상가용건비설탁법조방방약가감구복치료，료정위14 d。관찰증상개선정황、염성지표、뇨 pH 치、혈뇨산（ BUA）화림상안전성등지표변화정황。결과치료조총유효솔위92.31%，대조조위88.46%，량조료효비교，차이유통계학성의의（P ﹤0.05）；치료후량조환자적관절종통지수、VAS 평분、홍세포침강솔（ESR）、백세포（WBC）、초민 C 반응단백（hs - CRP）、중성립세포백분비（N%）、pH치등염증지표균명현하강（P ﹤0.05）；량조동통시간비교차이유통계학의의（P ﹤0.01）；여대조조비교，치료조무위장도불량반응（P ﹤0.01）、간신손해등명현불량반응。결론건비설탁방치료급성기통풍성관절염능구유효개선림상증상、강저혈뇨산、감화뇨액、축단료정，차림상안전성우우단순서약치료자。
Objective To observe the clinical efficacy on acute gouty arthritis treated with the ther-apy for strengthening the spleen and removing the turbid. Methods One hundred and four patients of acute gouty arthritis were randomized into two groups. In a control group(52 cases),the routine management with western medicine was used. In a treatment group(52 cases),on the basis of the routine management with western medicine,the modified formula for strengthening the spleen and removing the turbid was applied for oral administration. The duration of treatment was 14 days. The improvement in symptoms,inflammatory inde-xes,urine pH value,BUA and clinical safety were observed. Results The total effective rate was 92. 31% in the treatment group and was 88. 46% in the control group,indicating the significant difference in comparison (P ﹤ 0. 05). After treatment,the joint swelling and pain index,VAS score,ESR,WBC,hs - CRP,N% and pH value were all apparently reduced in the two groups(P ﹤ 0. 05). The difference in pain duration was not significant in comparison of the two groups(P ﹤ 0. 01). Compared with the control group,there was no gastro-intestinal adverse reaction in the treatment group(P ﹤ 0. 01)and no apparent adverse reaction of liver and Kidney damages. Conclusion The therapy for strengthening the spleen and removing the turbid effectively relieves the clinical symptoms of acute gouty arthritis,reduces blood uric acid and alKaline urine and shortens the treatment period. The clinical safety is better than that treated with simple western medicine.
