目的:探讨多层螺旋CT扫描在泌尿系统三维成像中的应用价值。方法:拟对在本院诊治的106例泌尿系统病变患者进行MSCTU平扫、增强、延迟扫描的病例进行回顾性整理,采用曲面重组法(CPR)、最大密度投影法(MIP)、多平面重组法(MPR )及容积成像法(VR)等三维重建技术对原始数据进行重建和分析。结果:MSCTU尿路成像能够清晰显示泌尿系统全貌,同时显示泌尿系统与附近结构和器官的相互关系。106例患者中,共检出泌尿系统结石71例,其中肾结石35例,输尿管结石20例,肾脏及输尿管多发结石16例;先天性发育畸形7例,包括3例双肾盂双输尿管畸形,2例巨输尿管,l例输尿管囊肿和1例马蹄形肾;肾囊肿4例,包括2例单侧囊肿,2例双侧囊肿;肾脏、输尿管、膀胱肿瘤14例,包括4例肾癌、5例输尿管癌和5例膀胱癌;肾脏创伤、肾内血肿3例;输尿管狭窄7例。结论:MSCTU扫描时间短,图像质量好,可获得不同层面、不同角度的三维图像,能良好地显示泌尿系统病变的位置、形态、解剖关系、毗邻关系、血供情况及性质,对泌尿系统疾病的诊断及临床治疗有独特的临床应用价值。

목적:탐토다층라선CT소묘재비뇨계통삼유성상중적응용개치。방법:의대재본원진치적106례비뇨계통병변환자진행MSCTU평소、증강、연지소묘적병례진행회고성정리,채용곡면중조법(CPR)、최대밀도투영법(MIP)、다평면중조법(MPR )급용적성상법(VR)등삼유중건기술대원시수거진행중건화분석。결과:MSCTU뇨로성상능구청석현시비뇨계통전모,동시현시비뇨계통여부근결구화기관적상호관계。106례환자중,공검출비뇨계통결석71례,기중신결석35례,수뇨관결석20례,신장급수뇨관다발결석16례;선천성발육기형7례,포괄3례쌍신우쌍수뇨관기형,2례거수뇨관,l례수뇨관낭종화1례마제형신;신낭종4례,포괄2례단측낭종,2례쌍측낭종;신장、수뇨관、방광종류14례,포괄4례신암、5례수뇨관암화5례방광암;신장창상、신내혈종3례;수뇨관협착7례。결론:MSCTU소묘시간단,도상질량호,가획득불동층면、불동각도적삼유도상,능량호지현시비뇨계통병변적위치、형태、해부관계、비린관계、혈공정황급성질,대비뇨계통질병적진단급림상치료유독특적림상응용개치。
Objective:To explore the application value of multi-slice spiral CT in urinary system of three-dimensional imaging.Method:The quasi on plain MSCTU scan, enhancement, delayed scanning in 106 cases of urinary system disease patients in our hospital were retrospectively sorting,the original data were analyzed using surface recombination method (CPR), maximum intensity projection (MIP), multi planar reconstruction (MPR) and volume rendering (VR) and other 3D reconstruction methods.Result:MSCTU urinary tract imaging can clearly display the urinary system, also showed a correlation between urinary system and nearby structures and organs.In 106 cases, 71 cases of urinary calculi were detected, including 35 cases of renal calculi, 20 cases of ureteral calculi, 16 cases of renal and ureteral multiple stones; and congenital malformation in 7 cases, including 3 cases of double renal pelvis and double ureter, 2 cases of megaureter, ureter cyst cases L and 1 cases of horse hoof shaped kidney; renal cysts in 4 cases, including 2 cases of unilateral cyst, 2 cases of bilateral cyst; kidney, ureter, bladder tumor in 14 cases, including 4 cases of renal cell carcinoma, 5 cases of ureteral carcinoma and 5 cases of bladder cancer; 3 cases of kidney trauma, renal hematoma; ureteral stenosis in 7 cases.Conclusion:MSCTU scan time is short, the image quality is good, the three-dimensional image can be obtained at different levels, from different angles, can be a good show urinary system disease location, morphology, anatomy, adjacent relations, the blood supply and properties for diagnosis and clinical treatment of diseases of the urinary system has the unique value in clinical application.
