目的 比较观察中成药参松养心胶囊和胺碘酮对心肌梗死家兔心肌微循环、心功能及心脏电生理特性的影响.方法 32只日本大耳兔随机分为假手术组,心梗组,心梗胺碘酮组,心梗参松组,每组8只.各组模型分别于术后2周、4周行心肌微泡造影(MCE).术后8周行心脏超声检查,并行在体电生理研究.结果 与心梗组比较,心梗胺碘酮组、心梗参松组术后2周、4周微血管密度(A)×血流速度(β)值均明显增大,且术后2周心梗参松组较心梗胺碘酮组增加更为明显;术后8周两组家兔心功能均有所改善,心梗参松组较心梗胺碘酮组改善更为明显;两组左心室正常区跨壁复极离散度(TDR)减小,梗死周边区与正常区区域离散度减小明显,心室颤动阈值(VFT)升高.结论 参松养心防治梗死后心律失常效应与胺碘酮差异无统计学意义.然其对心梗亚急性期微循环改善作用明显优于胺碘酮,对心功能的改善作用亦优于胺碘酮.

Objective To investigate the effects of Shen Song Yang Xin(SSYX)on heart function,myocardial microcirculation and electrophysiology characteristics in 8-week infarcted rabbit hearts and compared with amiodarone.Methods Thirty-two Japanese rabbits were randomly assigned to four groups:sham group (SO group),corresponding myocardial infarction (MI) control group (MI group),MI amiodarone group (AD group) and MI SSYX group(SSYX group).All animals underwent low-energy real-time myocardial contrast echocardiography (RT-MCE) at weeks 2 and 4 after coronary artery occlusion to assess the myocardial perfusion.Then,2-dimensional echocardiography(2DE)was applied eight weeks after operation for the cardiac function tests.Cardiac electrophysiological study was performed in vivo using the monophasic action potential technique.Results Compared with the MI group,both the AD group and the SSYX group displayed improved left ventricular function and gradually increased A×β values at two and four weeks post-procedure;however,the SSYX group presented a larger increase in these parameters.The two treated groups had similar electrophysiological characteristics,except that the AD group had a larger and longer MAPD90 in the normal zone,whereas the SSYX group displayed an inhibited prolongation of MAPD90 compared with the MI group.Conclusion Our studies indicate that SSYX has favorable advantages in improving left ventricular function and ameliorating the impaired myocardial microcirculation.SSYX has the same efficiency as the antiarrhythmic amiodarone after healing or healed MI,which might be closely associated with dredging collaterals and integration effects.
