目的:构建并评价实时荧光聚合酶链反应(PCR)熔解曲线法检测人类白细胞抗原-B27(HLA-B27)。方法通过采用本研究所构建的实时荧光 PCR 熔解曲线法和微量淋巴细胞毒法、实时荧光定量 PCR 法检测81例标本,并对3种方法进行评价。结果实时荧光 PCR 熔解曲线法与实时荧光定量 PCR 的敏感度和特异度均达到93.8%、90.9%,明显优于微量淋巴细胞毒法的敏感度56.2%和特异度81.8%。结论实时荧光 PCR 熔解曲线法具有很好的敏感度和特异度,具有较好的临床适应性。

Objective To establish and evaluate the technique of the real-time fluorescence quantitative polymerase chain reaction with DNA melting curve analysis for detecting the HLA-B27.Methods Three kinds of methods,including melting curve analysis,micro-complement dependent cytotoxicity test and real-time fluorescence PCR were established.HLA-B27 was detected in 81 patients by the different menthods.Results The sensitivity and specificity of melting curve and real-time fluorescence PCR reached 93.8% and 90.9%,respectively.Those of micro-complement dependent cytotoxicity test reached 56.2% and 81.8%,respectively.Sensitivity and specificity were significantly higher in melting curve and real-time fluorescence PCR than in micro-complement dependent cytotoxicity test.Conclusion Real-time fluorescence PCR with DNA melting curve analysis show high sensitivity and specificity and may be hepful for clinical diagnosis.
