目的:探讨超低剂量 CT 在体检人群中肺结节体积随诊中的应用价值。方法行常规剂量胸部 CT 扫描遵医嘱半年后复查的125例体检者共145例实性结节,同时行50 mAs 和25 mAs 低剂量与超低剂量2次胸部 CT扫描所获得的影像资料进行分析。结节体积应用 GE4.4后处理工作站三维体积测量软件(advanced lung analysis, ALA)自动进行测量。计算2次测量的相对体积差值(relative volume difference,RVD),RVD≤10%为体积测量满意, RVD>10%为体积测量不满意。结果145例结节2次体积测量的满意度为77.9%,相关系数 r 为0.997,体积测量差异无统计学意义(P >0.05);Bland-Altman 散点图 RVD 的95%可信区间为-0.13~0.15,2次体积测量总体趋于一致。两样本 t 检验显示毛刺型结节及组织旁型结节 RVD 不满意率较其他类型结节明显增高。结论超低剂量 CT相对于低剂量 CT 三维体积测量具有较高的一致性,可用于肺结节的体积随诊工作中。而对于毛刺型结节及胸膜旁型结节,我们建议采用常规低剂量 CT 进行随访观察。

Objective To evaluate the application value of ultra low dose CT with three-dimensional volumetric measurement in follow-up physical examination population with solid pulmonary nodules.Methods The study was performed on the data of 145 solid nodules in 125 patients who had undergone a standard-dose chest CT scan and had a follow-up 25mAs and 50mAs CT simultaneously.The volume of pulmonary nodules was measured using advanced lung analysis software.The level of agreement was expressed using the relative volume differences.Results The ratio of satisficing segmentation was 77.9% for 145 nodules.Spearman correlation efficient between two conditions was highly correlated (r =0.997).The repeated measurements did not result in significant difference (P >0.05).Bland-Altman scatter diagram revealed that 95% CI of RVD was -0.13-0.1 5.Two paired samples T test showed that spiculated and pleura-attached nodules had significantly high ratio of unsatisfied segmentation respectively.Conclusion Three-dimensional measurement of ultra low dose CT shows high consistency and can be used for daily follow-up work of pulmonary nodules.However,a routine low dose was advised for spiculated and pleura-attached nodules.
