目的:应用二维斑点追踪技术对 H 型高血压及非 H 型原发性高血压患者心脏机能进行对比性研究。方法 H 型高血压组49例,非 H 型原发性高血压组43例,以及正常对照组60例,分别对观察者的左心室射血分数、左心房内径、左心室内径,16节段室壁的纵向应变,径向应变,环状应变进行测量。结果 H 型高血压组及非 H 型高血压组与对照组纵向应变、径向应变及环状应变差异均有统计学意义(P <0.05),纵向应变的变化较为明显;H 型高血压组与非 H 型高血压组之间比较,部分室壁的纵向应变、径向应变及环状应变差异均有统计学意义(P <0.05)。结论 H 型高血压及非 H 型高血压均可引起心脏收缩功能的减低,且 H 型高血压更为明显,说明高同型半胱氨酸对冠状动脉的损害更为明显。

목적:응용이유반점추종기술대 H 형고혈압급비 H 형원발성고혈압환자심장궤능진행대비성연구。방법 H 형고혈압조49례,비 H 형원발성고혈압조43례,이급정상대조조60례,분별대관찰자적좌심실사혈분수、좌심방내경、좌심실내경,16절단실벽적종향응변,경향응변,배상응변진행측량。결과 H 형고혈압조급비 H 형고혈압조여대조조종향응변、경향응변급배상응변차이균유통계학의의(P <0.05),종향응변적변화교위명현;H 형고혈압조여비 H 형고혈압조지간비교,부분실벽적종향응변、경향응변급배상응변차이균유통계학의의(P <0.05)。결론 H 형고혈압급비 H 형고혈압균가인기심장수축공능적감저,차 H 형고혈압경위명현,설명고동형반광안산대관상동맥적손해경위명현。
Objective Two-dimensional speckle tracking imaging was applied in comparative research on the heart function of H and non-H hypertension patients.Methods The study involved 49 cases of H hypertension,43 cases of non-H essential hypertension and 60 normal control cases,their left ventricular ejection fraction,the size of left atria,and left ventricula,longitudinal strain,radial strain and circumferential strain of 1 6 segmental walls were measured.Results H hypertension patients and non-H hypertension patients showed significant difference in longitudinal strain,radial strain and circumferential strain when compared with controlled group,among which the longitudinal strain showed the most obvious change.While compared H hypertension patients with non-H hypertension patients,the longitudinal strain,radial strain and circumferential strain all showed statistical significance(P <0.05). Conclusion H hypertension and non-H hypertension both result in the reduction of heart systolic function,especially among H hypertension patients,which shows the harmfulness of homocysteine to coronary artery is more obviously serious.
