目的：探讨不同产程给药硬膜外分娩镇痛的效果。方法选择2012年4月~2013年4月于本院分娩的144例足月初产妇作为研究对象，将其随机分为实验组和对照组。实验组采用潜伏期硬膜外分娩镇痛，对照组采用活跃期硬膜外分娩镇痛，比较两组的满意度、VAS 评分、产程进展、分娩方式、母婴情况以及不良反应发生情况。结果实验组产妇满意度优秀率高于对照组，潜伏期、VAS评分低于对照组，差异有统计学意义(P<0.05)。两组产妇产程进展、顺产率、剖宫产率、产后出血量、新生儿Apgar评分和不良反应发生率比较差异无统计学意义(P>0.05)。实验组器械助产率高于对照组，差异有统计学意义(P<0.05)。结论潜伏期镇痛效果较好，可显著提高产妇满意度，对产程和分娩方式并无明显影响。

목적：탐토불동산정급약경막외분면진통적효과。방법선택2012년4월~2013년4월우본원분면적144례족월초산부작위연구대상，장기수궤분위실험조화대조조。실험조채용잠복기경막외분면진통，대조조채용활약기경막외분면진통，비교량조적만의도、VAS 평분、산정진전、분면방식、모영정황이급불량반응발생정황。결과실험조산부만의도우수솔고우대조조，잠복기、VAS평분저우대조조，차이유통계학의의(P<0.05)。량조산부산정진전、순산솔、부궁산솔、산후출혈량、신생인Apgar평분화불량반응발생솔비교차이무통계학의의(P>0.05)。실험조기계조산솔고우대조조，차이유통계학의의(P<0.05)。결론잠복기진통효과교호，가현저제고산부만의도，대산정화분면방식병무명현영향。
Objective To discuss the effect of epidural analgesia given medicine in different stage of labor. Methods 144 full term primiparae delivered in our hospital from April 2012 to April 2013 were chosen as study objects,and they were divided into experimental group and control group.Experimental group was given epidural analgesia during incubation period,while control group was epidural analgesia during active period.Satisfaction degree,VAS score,birth process,mode of delivery,mother and baby condition and adverse reaction between two groups was compared respec-tively. Results The excellent rate of satisfaction degree for delivery women in experimental group was higher than that of control group,VAS score during incubation period was lower than that of control group,and there was a statistical dif-ference (P<0.05).There was no statistical difference of birth process,rate of vaginal delivery and cesarean section rate, amount of postpartum hemorrhage,neonatal Apgar score and the incidence rate of adverse reaction between two groups (P>0.05).Instrumental delivery rate of experimental group was higher than that of control group,and there was a statisti-cal difference (P<0.05). Conclusion Analgesic effect during incubation period is better and it can obviously improve satisfaction degree of delivery woman,and It has no apparent influence on birth process and mode of delivery.
