目的:探讨高铝暴露对人体胰岛素分泌功能、糖代谢及白细胞介素-6(IL-6)、C反应蛋白(CRP)的影响及相关性。方法高铝暴露区健康成人居民61名,分别测定体重指数(BMI)、收缩压(SBP)、舒张压(DBP)、血清铝元素(Al)浓度、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、肌酐(Cr)、促甲状腺激素(TSH)、皮质醇、IL-6、CRP等,计算稳态模型胰岛β细胞功能指数(HOMA-β),并与63名非高铝暴露区健康成人进行比较。结果与非高铝暴露区居民比较,高铝暴露区居民Al、FBG、HbA1c、IL-6、CRP水平升高,而FINS、HOMA-β水平降低(P<0.05或P<0.01);相关性分析显示,FBG、IL-6、CRP与血Al水平呈正相关(r=0.464、0.690、0.850,均P<0.01),HOMA-β与Al水平呈负相关(r=-0.331,P<0.01)。结论长期高铝暴露可导致机体胰岛β细胞分泌功能下降及糖代谢紊乱,其机制可能与机体炎性反应增强有关。

목적:탐토고려폭로대인체이도소분비공능、당대사급백세포개소-6(IL-6)、C반응단백(CRP)적영향급상관성。방법고려폭로구건강성인거민61명,분별측정체중지수(BMI)、수축압(SBP)、서장압(DBP)、혈청려원소(Al)농도、공복혈당(FBG)、당화혈홍단백(HbA1c)、공복이도소(FINS)、곡병전안매(ALT)、곡초전안매(AST)、기항(Cr)、촉갑상선격소(TSH)、피질순、IL-6、CRP등,계산은태모형이도β세포공능지수(HOMA-β),병여63명비고려폭로구건강성인진행비교。결과여비고려폭로구거민비교,고려폭로구거민Al、FBG、HbA1c、IL-6、CRP수평승고,이FINS、HOMA-β수평강저(P<0.05혹P<0.01);상관성분석현시,FBG、IL-6、CRP여혈Al수평정정상관(r=0.464、0.690、0.850,균P<0.01),HOMA-β여Al수평정부상관(r=-0.331,P<0.01)。결론장기고려폭로가도치궤체이도β세포분비공능하강급당대사문란,기궤제가능여궤체염성반응증강유관。
Objective To investigate the influence and correlation of high aluminum exposure on insulin secretion function of body and glucose metabolism and interleukin-6 (IL-6) and C reactive protein (CRP). Methods Sixty one cases of healthy adults of high aluminum exposed area were respectively taken to determine the body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure (DBP), the serum aluminum concentration of elements (Al), fast-ing blood glucose (FBG), hemoglobin A1c (HbA1c), fasting insulin (FINS), alanine aminotransferase (ALT), aspartate aminotransferase (AST), creatinine (Cr), thyroid stimulating hormone (TSH), cortisol, IL-6, CRP and so on. And home-ostasis model assessment β-cell function indexes (HOMA-β) were calculated, then it was compared with 63 cases of healthy adults of non high aluminum exposed area. Results Compared with the resident of non high aluminum exposed area, the levels of Al, FBG, HbA1c, IL-6 and CRP in the resident of high aluminum exposed area increased, but the levels of FINS and HOMA-βdecreased (P<0.05 or P<0.01);the correlation analysis showed that the levels of FBG, IL-6 and CRP had positive correlation with serum Al (r = 0.464, 0.690, 0.850, all P< 0.01), the level of HOMA-βhad a negative correlation with Al (r=-0.331, P<0.01). Conclusion The pancreatic islet β cells function of organism descended and glucose metabolic disordered can be caused by long-term high aluminum exposure, its mechanism maybe related to strengthening the inflammatory response of organism.
