目的:观察静脉注射利多卡因在全麻诱导气管插管期间对老年糖尿病患者心率变异性的影响。方法:将86例择期进行腹部手术的老年糖尿病患者,按随机分组双盲法分为两组:A组(1.5mg/kg利多卡因组,n=43)和B组(1.5mg/kg生理盐水组,n=43)。应用功率谱分析技术于麻醉前(T0)、麻醉诱导后(T1)及气管插管后(T3)观察患者的心率变异性。结果:T0时,LF、HF、LF/HF、TP两组间比较均无明显差异;T1时,与B组相比,A组LF呈降低趋势,HF明显升高(P<0.05);T2时,A组LF、LF/HF较B组显著下降(P<0.01)。结论:利多卡因能有效降低气管插管对心率变异性的影响,对老年糖尿病患者围插管期有明显的自主神经保护功能。

목적:관찰정맥주사리다잡인재전마유도기관삽관기간대노년당뇨병환자심솔변이성적영향。방법:장86례택기진행복부수술적노년당뇨병환자,안수궤분조쌍맹법분위량조:A조(1.5mg/kg리다잡인조,n=43)화B조(1.5mg/kg생리염수조,n=43)。응용공솔보분석기술우마취전(T0)、마취유도후(T1)급기관삽관후(T3)관찰환자적심솔변이성。결과:T0시,LF、HF、LF/HF、TP량조간비교균무명현차이;T1시,여B조상비,A조LF정강저추세,HF명현승고(P<0.05);T2시,A조LF、LF/HF교B조현저하강(P<0.01)。결론:리다잡인능유효강저기관삽관대심솔변이성적영향,대노년당뇨병환자위삽관기유명현적자주신경보호공능。
Objective:To study the effects of lidocaine on heart rate variability associated with endotracheal intubation during general anesthesia in the elderly with diabetes. Methods:eighty-six elderly patients scheduled for abdominal surgery with endotracheal intubation under general anesthesia were randomly divided into two groups: group A (1.5mg/kg lidocaine group, n=43)and group B(1.5mg/kg sodium chloride solution,n=43) . Use power spectral analysis , the heart rate variability (HRV) was tested at three time points :before anesthesia (T0),behind anesthesia (T1)and after endotracheal intubation (T2) . Results:The index of LF,HF,LF/HF,TP all have no difference between two groups at T0; But at T1, Compared with group B, LF was significantly lowerand HF was higher in group A (P<0.05); However, LF, LF/HF in group A was lower than group B at T2. Conclusion:Lidocaine can effectively reduce the heart rate variability during endotracheal intubation. Lidocaine is beneficial to maintain the function of the cardiac autonomic nerve in diabetes patients.
