目的：探讨锥颅血肿抽吸术联合开颅血肿清除术治疗外伤性颅脑损伤的临床价值。方法62例外伤性颅脑损伤患者，随机分为观察组(33例)和对照组(29例)。对照组给予传统开颅血肿清除术治疗，观察组在对照组治疗基础上给予锥颅血肿抽吸术，观察两组患者治疗疗效。结果观察组患者术后1、2、4周格拉斯哥昏迷评分(GCS评分)均显著低于对照组，差异有统计学意义(P<0.05)；观察组患者治疗后Barthel指数显著高于对照组，差异有统计学意义(P<0.05)。结论锥颅血肿抽吸术联合开颅血肿清除术治疗外伤性颅脑损伤可提高血肿清除率，减轻昏迷状态，改善预后，具有较好的应用前景。
목적：탐토추로혈종추흡술연합개로혈종청제술치료외상성로뇌손상적림상개치。방법62예외상성로뇌손상환자，수궤분위관찰조(33례)화대조조(29례)。대조조급여전통개로혈종청제술치료，관찰조재대조조치료기출상급여추로혈종추흡술，관찰량조환자치료료효。결과관찰조환자술후1、2、4주격랍사가혼미평분(GCS평분)균현저저우대조조，차이유통계학의의(P<0.05)；관찰조환자치료후Barthel지수현저고우대조조，차이유통계학의의(P<0.05)。결론추로혈종추흡술연합개로혈종청제술치료외상성로뇌손상가제고혈종청제솔，감경혼미상태，개선예후，구유교호적응용전경。
ObjectiveTo investigate the clinical value of transcranial haematoma aspiration combined with craniotomy hematoma clearance in the treatment of traumatic brain injury.MethodsA total of 62 patients with traumatic brain injury were randomly divided into observation group (33 cases) and control group (29 cases). The control group received traditional craniotomy hematoma clearance, and the observation group received additional transcranial haematoma aspiration. Curative effects of the two groups were compared.ResultsThe observation group had much lower Glasgow coma scale (GCS scores) in 1, 2, 4 weeks after operation than the control group, and their difference had statistical significance (P<0.05). The observation group had obviously higher Barthel score after treatment than the control group, and their difference had statistical significance (P<0.05).ConclusionCombination of transcranial haematoma aspiration and craniotomy hematoma clearance can increase hematoma clearance rate, reduce comatose state, and improve prognosis. This method contains bright prospect of application.
