脑卒中是神经内科常见疾病之一，具有发病率高、复发率高、病死率高和致残率高的特点。重组组织型纤维蛋白酶原激活剂(rt-PA)是自1996年以来唯一被美国食品和药物管理局(FDA)批准用于急性缺血性脑卒中(AIS)溶栓治疗的药物，但因静脉溶栓的治疗时间窗狭窄(<4.5 h)、大血管再通率低及出血风险限制了rt-PA的使用，导致全球范围内的溶栓治疗率一直很低。近年，相关血管内治疗技术有了快速发展，显著提高了闭塞血管的开通率。文中就静脉溶栓、动脉溶栓、机械取栓、机械碎栓、支架成形等治疗技术的相关情况进行介绍。

Ischemic stroke is one of the common diseases in department of internal medicine, with high incidence rate, high recurrence rate, high mortality rate and high disability rate. Recombinant tissue plasminogen activator is the only drug treatment for acute ischemic stroke (AIS) approved by the United States Food and Drug Administration (FDA) since 1996. But the narrow time window(<4.5 h), a lower probability of recanalization and the risk of hemorrhage restrict the use of rt-PA. As an alternative treatment, the endovascular treatment technology has been advanced greatly in recent years, signiifcantly improved the rate of occlusive vascular recanalization. The progress for intravenous thrombolysis, intra-arterial thrombolysis, mechanical thrombectomy, mechanical thrombectomy and stent angioplasty were reviewed.
