目的:各种原因导致的食管瘘的临床治疗较为棘手,本研究旨在探讨可回收覆膜食管支架封堵瘘口的疗效和安全性。方法2008年1月至2012年3月新疆维吾尔自治区人民医院胸外科共收治食管瘘患者47例,其中食管气管瘘23例,食管癌、贲门癌术后吻合口瘘11例,自发性食管破裂9例,食管肺瘘2例,食管纵隔瘘2例。于胃镜下置入可回收覆膜食管支架45例,并观察疗效。结果所有食管支架均一次性成功置入,无严重并发症发生。35例达到完全封堵,10例达到部分封堵。1个月内有3例支架出现不同程度移位,1例脱落至胃内,1例脱落至肠腔内。治疗时间5~85 d,平均治疗时间34 d。结论可回收覆膜食管支架能有效封堵食管瘘口,延长患者生命和提高生活质量。

목적:각충원인도치적식관루적림상치료교위극수,본연구지재탐토가회수복막식관지가봉도루구적료효화안전성。방법2008년1월지2012년3월신강유오이자치구인민의원흉외과공수치식관루환자47례,기중식관기관루23례,식관암、분문암술후문합구루11례,자발성식관파렬9례,식관폐루2례,식관종격루2례。우위경하치입가회수복막식관지가45례,병관찰료효。결과소유식관지가균일차성성공치입,무엄중병발증발생。35례체도완전봉도,10례체도부분봉도。1개월내유3례지가출현불동정도이위,1례탈락지위내,1례탈락지장강내。치료시간5~85 d,평균치료시간34 d。결론가회수복막식관지가능유효봉도식관루구,연장환자생명화제고생활질량。
Objective The aim of this study is to evaluate the safety and clinical efficacy of recyclable coated esophageal stent for the treatment of esophageal fistula.Methods 47 patients with esophageal fistula including 11 cases of anastomotic fistula after operation,23 esophagorespiratory fistula,2 esophagoplumonary fistula,9 spontaneous rupture of the esophagus and 2 esophagomediastinal fistula between Jan.2008 and Mar.2012 had been studied,45 esophageal stents were placed under gastroscopy. The clinical efficacy was observed.Results All stents were successfully placed only once without serious complication.35 cases showed complete sealing effect,10 showed part effect.The stents shifting occurred in different degree in 3 cases in one month,1 off to the stomach and 1 to the intestine.The treatment time was 5 to 58 days,and an average of 34 days.Conclusion The treatment of recyclable coated esophageal stent placement for esophageal fistula can effectively close fistula to prolong survival time and improve quality of life.
