探讨血浆B型尿钠肽(BNP)、高敏肌钙蛋白T(hs‐cTnT)在新生儿持续肺动脉高压(PPHN)心肌损伤早期诊断和治疗中的应用。本研究以60例 PPHN 患儿和40例正常新生儿为研究对象,对BNP、hs‐cTnT 的水平与肺动脉收缩压(PASP)的相关性进行研究。结果表明,PPHN组患儿BNP、hs‐cTnT 水平显著高于正常新生儿(P<0.05)。治疗2周后,患儿BNP、hs‐cTnT水平均较治疗前明显降低(P<0.05)。BNP、hs‐cTnT 水平与 PASP呈显著正相关(P<0.01)。BNP的敏感性及特异性分别为88.36%、90.41%,而hs‐cTnT的敏感性及特异性分别为82.67%、85.29%。因此,BNP、hs‐cT nT可作为心肌损伤早期诊断和疗效评估的重要指标。

탐토혈장B형뇨납태(BNP)、고민기개단백T(hs‐cTnT)재신생인지속폐동맥고압(PPHN)심기손상조기진단화치료중적응용。본연구이60례 PPHN 환인화40례정상신생인위연구대상,대BNP、hs‐cTnT 적수평여폐동맥수축압(PASP)적상관성진행연구。결과표명,PPHN조환인BNP、hs‐cTnT 수평현저고우정상신생인(P<0.05)。치료2주후,환인BNP、hs‐cTnT수평균교치료전명현강저(P<0.05)。BNP、hs‐cTnT 수평여 PASP정현저정상관(P<0.01)。BNP적민감성급특이성분별위88.36%、90.41%,이hs‐cTnT적민감성급특이성분별위82.67%、85.29%。인차,BNP、hs‐cT nT가작위심기손상조기진단화료효평고적중요지표。
To investigate the application of plasma brain‐type natriuretic peptide and high sensitivity of cardiac troponin T for early diagnose and treatment in myocardium injure of persistent pulmonary hypertension of the newborn (PPHN) .In this study ,60 PPHN and 40 normal neonates were chosen to investigate the correlation of BNP ,hs‐cTnT and pulmonary artery systolic pressure . The results showed that plasma levels of BNP , hs‐cTnT in case group were statistically significantly higher than those in control group (P< 0 .05) .After being treated for two weeks ,the levels of BNP ,hs‐cTnT dropped significantly compared with that before treatment .There was a positive correlation between the plasma levels of BNP ,hs‐cTnT and PASP .The sensibility and specificity of BNP were 88 .36% and 90 .41% ,while those for hs‐cTnT were 82 .67% and 85 .29% .Therefore ,early PPHN may onset by myocardium injure ,and the BNP ,hs‐cTnT were important indexes for early diagnose and treatment in myocardium injure of persistent pulmonary hypertension of the newborn .
