目的:探讨超声造影在肝脏恶性肿瘤微波消融治疗后近期疗效评估的应用价值。方法选取48例肝癌患者(病灶共58个),微波消融治疗后1个月进行复查,分别采用CDFI、CECT、CEUS三种方法进行诊断。结果微波消融治疗后1个月,肝脏未发现存在新的肿瘤病灶,未发现出血、感染、胆瘘等并发症,CDFI诊断准确率明显低于CECT准确率、CEUS准确率(63.79% vs 89.66% vs 94.83%);CECT准确率与CEUS准确率比较无明显差异(89.66%vs 94.83%)。结论超声造影具有实时观察、简便的特点,对于诊断肝脏肿瘤残存具有较高的准确性,是早期评价微波消融的有效方法,可成为肝癌局部治疗随访的重要手段。

목적:탐토초성조영재간장악성종류미파소융치료후근기료효평고적응용개치。방법선취48례간암환자(병조공58개),미파소융치료후1개월진행복사,분별채용CDFI、CECT、CEUS삼충방법진행진단。결과미파소융치료후1개월,간장미발현존재신적종류병조,미발현출혈、감염、담루등병발증,CDFI진단준학솔명현저우CECT준학솔、CEUS준학솔(63.79% vs 89.66% vs 94.83%);CECT준학솔여CEUS준학솔비교무명현차이(89.66%vs 94.83%)。결론초성조영구유실시관찰、간편적특점,대우진단간장종류잔존구유교고적준학성,시조기평개미파소융적유효방법,가성위간암국부치료수방적중요수단。
Objective To explore the application value of contrast enhanced ultrasound in the evaluation of recent curative effect for liver malignant tumor patients who received the treatment of microwave ablation . Methods 48 patients with hepatocellular carcinoma ( 58 lesions ) were checked about one month after the treatment of microwave ablation . Three kinds of method ( CDFI, CECT, CEUS ) were used to diagnose respectively .Results After 1 months of treatment by microwave ablation , no new tumor lesions were found in theliver, as well as no bleeding , infection, biliary fistula and other complications occurred .The accuracy rate of CDFI diagnosis was significantly lower than that of CEUS and CECT (63.79%VS 89.66%and 94.83%).The difference of accuracy rate between CECT and CEUS has no statistically significant (89.66% VS94.83%). Conclusions Contrast enhanced ultrasound has the characteristics of real -time observation and simplicity and has higher accuracy in the diagnosis of hepatic tumor residual .It is an effective method for early evaluation of microwave ablation , can be an important means for following -up of liver cancer local treatment .
