目的:研究COX-2和Ki67在胆囊上皮内瘤变及胆囊癌组织中的表达,探讨二者在胆囊癌发展中的作用,以及在胆囊上皮内瘤变和胆囊癌诊断中的意义。方法应用免疫组化技术SP法检测54例胆囊低级别上皮内瘤变,45例高级别上皮内瘤变及39例胆囊癌标本中COX-2和Ki67的表达情况。结果 COX-2和Ki67在胆囊低级别上皮内瘤变、高级别上皮内瘤变和胆囊癌组织中的阳性表达率分别为25.93%(14/54)、48.89%(22/45)和76.92%(30/39)与22.22%(12/54)、35.66%(16/45)和74.36%(29/39)。二者随胆囊黏膜异型性增加,阳性表达率均明显升高,具有明显的差异(P<0.01)。各组间COX-2和Ki67表达呈正相关,r=0.924(P<0.05)。结论 COX-2和Ki67可作为鉴别胆囊上皮内瘤变和胆囊癌的指标,可能与胆囊癌的发生发展有关。

목적:연구COX-2화Ki67재담낭상피내류변급담낭암조직중적표체,탐토이자재담낭암발전중적작용,이급재담낭상피내류변화담낭암진단중적의의。방법응용면역조화기술SP법검측54례담낭저급별상피내류변,45례고급별상피내류변급39례담낭암표본중COX-2화Ki67적표체정황。결과 COX-2화Ki67재담낭저급별상피내류변、고급별상피내류변화담낭암조직중적양성표체솔분별위25.93%(14/54)、48.89%(22/45)화76.92%(30/39)여22.22%(12/54)、35.66%(16/45)화74.36%(29/39)。이자수담낭점막이형성증가,양성표체솔균명현승고,구유명현적차이(P<0.01)。각조간COX-2화Ki67표체정정상관,r=0.924(P<0.05)。결론 COX-2화Ki67가작위감별담낭상피내류변화담낭암적지표,가능여담낭암적발생발전유관。
Objective To investigate the expression of COX -2 and Ki67 proteins in cholecystic intraepithelial neoplasia and gallbladder carcinoma , their effect on development of gallbladder carcinoma , the significance in diagnosing cholecystic intraepithelial neoplasia and gallbladder carcinoma . Methods Immunohistochemistry was used to detect the expression of Ki 67 and COX-2 in 54 cases of low grade cholecystic intraepithelial neoplasia (LCCIN),45 cases of high grade cholecystic intraepithelial neoplasia (HCCIN) and 39 cases of gallbladder carcinoma ( GBC).Results The positive expression rates of COX -2 protein in LCCIN, HCCIN and GBC were 25.93%, 48.89% and 76.92% respectively.The positive expression rates of Ki67 protein in LCCIN, HCCIN and GBC were 22.22%, 35.66%and 74.36%respectively.The expression of COX-2 and Ki67 increased as the growth of cholecystic epithelial atypia .There was a positive relationship between expression of COX-2 proteins and Ki67 proteins (r=0.924 P<0.05).Conclusions COX-2 and Ki67 mightbe potential evaluation marker for diagnose of CCIN , and GBC, and might be correlated to the development of gallbladder carcinoma .
