目的：比较传统护理与快速康复护理干预在腹腔镜全胃根治性切除术患者术后恢复中的影响，分析护理特点，提供有效的护理措施，在临床工作中践行快速康复理念。方法随机选取深圳市人民医院胃肠外科2013年1月至2014年6月80例腹腔镜全胃根治性切除术患者，随机分为观察组与对照组，每组40例。对照组采用传统护理；观察组遵循快速康复理念，在留置及拔除胃管时间、术后物理治疗、功能锻炼等方面进行快速康复护理干预；比较两组术后首次排气时间、进食时间、住院时间、短期并发症发生率等指标。结果观察组术后首次排气时间缩短、进食时间提前、住院总时间缩短、消化道不适症状及并发症发生率降低，与对照组比较，差异有统计学意义（P ＜0．05）。结论对腹腔镜全胃根治性切除术患者进行术后快速康复护理干预可提高临床疗效，缩短住院时间，降低术后短期并发症。
목적：비교전통호리여쾌속강복호리간예재복강경전위근치성절제술환자술후회복중적영향，분석호리특점，제공유효적호리조시，재림상공작중천행쾌속강복이념。방법수궤선취심수시인민의원위장외과2013년1월지2014년6월80례복강경전위근치성절제술환자，수궤분위관찰조여대조조，매조40례。대조조채용전통호리；관찰조준순쾌속강복이념，재류치급발제위관시간、술후물리치료、공능단련등방면진행쾌속강복호리간예；비교량조술후수차배기시간、진식시간、주원시간、단기병발증발생솔등지표。결과관찰조술후수차배기시간축단、진식시간제전、주원총시간축단、소화도불괄증상급병발증발생솔강저，여대조조비교，차이유통계학의의（P ＜0．05）。결론대복강경전위근치성절제술환자진행술후쾌속강복호리간예가제고림상료효，축단주원시간，강저술후단기병발증。
Objective To compare the effect on postoperative recover for the laparoscopic radical total gastrectomy between traditional nursing and fast track surgery nursing,analyze the nursing characters, propose effective nursing measures and practice the fast track surgery theory clinically.Methods From Jan.2013 to Jun.2014,80 patients with laparoscopic radical total gastrectom from Department of Gastrointestinal surgery Shenzhen People′s Hospital were randomly taken and separated into observed group and compared group randomly,40 patients in each group.Traditional nursing was used for compared group while fast track surgery idea was used for observed group and fast track surgery nursing intervention was taken on the gastric tube indwelling time and removing time,postoperative physical treatment and function training. Several indexes from both group were compared,such as first time to release gas,feeding time,length of hospital stay and incidence of short term complications.Results Compared with compared group,first time to release gas of observed group was earlier,feeding time was earlier too,length of hospital stay was shorter and incidence of short term complications was less.It made statistic sense with the compare results (P <0.05).Conclusions Postoperative fast track surgery nursing for the laparoscopic radical total gastrectomy will improve the clinical therapy effect and reduce length of hospital stay and incidence of short term complications.
