Sleep disorder breathing%Altitude
目的:探讨不同海拔高度对睡眠呼吸暂停的影响。方法应用便携式睡眠呼吸监测仪测定5名受试者(均为睡眠呼吸暂停患者)在不同海拔高度(500 m、1000 m、2261 m、2800 m、3650 m、4200 m)下的夜间呼吸、脉搏、动脉氧饱和度及晨起血压。结果与海拔500 m 相比,当海拔升至4200 m时,患者睡眠时的呼吸暂停低通气指数、氧减指数、低通气指数均升高[(28.6±11.3)次/h vs (11.0±5.4)次/h;(29.8±11.0)次/h vs (11.2±5.2)次/h;(19.6±9.0)次/h vs (7.6±3.4)次/h,P 值均<0.05]。不同海拔组间患者的其它指标如平均动脉氧饱和度、最低动脉氧饱和度、收缩压、舒张压等的变化也均有统计学意义(P 值均<0.05)。结论高海拔对睡眠呼吸紊乱的影响以中枢性睡眠呼吸暂停和低通气增高为主,应重视高海拔对睡眠呼吸暂停的影响。

목적:탐토불동해발고도대수면호흡잠정적영향。방법응용편휴식수면호흡감측의측정5명수시자(균위수면호흡잠정환자)재불동해발고도(500 m、1000 m、2261 m、2800 m、3650 m、4200 m)하적야간호흡、맥박、동맥양포화도급신기혈압。결과여해발500 m 상비,당해발승지4200 m시,환자수면시적호흡잠정저통기지수、양감지수、저통기지수균승고[(28.6±11.3)차/h vs (11.0±5.4)차/h;(29.8±11.0)차/h vs (11.2±5.2)차/h;(19.6±9.0)차/h vs (7.6±3.4)차/h,P 치균<0.05]。불동해발조간환자적기타지표여평균동맥양포화도、최저동맥양포화도、수축압、서장압등적변화야균유통계학의의(P 치균<0.05)。결론고해발대수면호흡문란적영향이중추성수면호흡잠정화저통기증고위주,응중시고해발대수면호흡잠정적영향。
Objective To explore the effect of different altitude on the patients with sleep apnea-hypopnea syndrome.Methods Five subjects underwent respiration and pulse oximeter tests.Apnea-hypopnea index (AHI),hypopnea index (HI),mean saturation of arterial blood oxygen (MSaO 2 ),the lowest saturation of arterial blood oxygen (LSaO 2 ),the time spent at SaO 2 below 90% (SIT90 ),the number of desaturations≥4% per hour(ODI4 )and blood pressure were calculated at different altitude (include 500 m,1 000 m,2 261 m,2 800 m,3 650 m and 4 200 m).Results The apnea-hypopnea index, hypopnea index,oxygen desaturation index,MSaO 2 ,LSaO 2 ,systolic pressure and diastolic pressure were significantly different among five different altitude groups (all P <0.05).The AHI rised from (1 1.0 ± 5.4)times/h to (28.6±1 1.3)times/h,the HI rised from (7.6±3.4)times/h to (1 9.6 ±9.0)times/h and ODI4 rised from (1 1.2±5.2)times/h to (29.8±1 1.0)times/h as altitude increases from 500 m to 4 200 m.Conclusions The influence of highter altitude on the respiratory function are predominantly central apneas and hypopneas.We should pay attention the effects of high altitude on the sleep apnea.
