目的：探索一种安全、高效、简单的方面改形技术并评价其远期疗效。方法：选择21例方脸求美者，在局部肿胀麻醉下经小切口用三步法完成手术，用注射器吸脂法作腮腺区皮下脂肪减薄，经口外小切口作直视下嚼肌高频热凝部分毁损术，在骨膜下作下颌角弧形切骨和外板部分切除术。最后用8-0尼龙线作切口精细美容缝合。结果：本组求美者全部获得成功，未出现术中和术后并发症。1年后随访16例，均获得理想卵圆面型，手术切口无明显瘢痕形成。结论：经口外微创切口作下颌角部脂肪、肌层和骨骼综合减容术可获得理想改型效果，切口无明显瘢痕形成，操作中不会损伤面神经，结果证实该技术是简单、安全、高效手术方法。
목적：탐색일충안전、고효、간단적방면개형기술병평개기원기료효。방법：선택21례방검구미자，재국부종창마취하경소절구용삼보법완성수술，용주사기흡지법작시선구피하지방감박，경구외소절구작직시하작기고빈열응부분훼손술，재골막하작하합각호형절골화외판부분절제술。최후용8-0니룡선작절구정세미용봉합。결과：본조구미자전부획득성공，미출현술중화술후병발증。1년후수방16례，균획득이상란원면형，수술절구무명현반흔형성。결론：경구외미창절구작하합각부지방、기층화골격종합감용술가획득이상개형효과，절구무명현반흔형성，조작중불회손상면신경，결과증실해기술시간단、안전、고효수술방법。
Objective To investigate a simple,safe and effective surgical correction for the Square Faces and evaluate the long term results. Methods 21 cases of patients with Square Faces were chosen for this study.The operations were carried out through extraoral approach under local tumescent anesthesia and the incision was less than 2.5cm long.The surgical procedures were divided into three steps,including syringe liposuction in the parotid gland area,partial masseter muscle damage with high frequency electric coagulation device and prominent mandibular angle curved-type osteotomy combined with the lateral cortex osteotomy around the mandibular angle.The incisions were closed with 8-0 nylon thread in aesthetic principle. Results All patients got good recover from the successful operations and no early complications were occurred in this group. 16 patients were followed up one year postoperation and they are all obtained ideal outcomes. 32 skin incisions of the 16 customers showed no scar in the neck skin. Conclusion Comprehensive correction for square to oven face transfer by extraoral mini invasive incisions could obtain ideal outcomes and the extraoral incisions could get scar-free healing without nerve damage.The long term results prove that the surgical procedure in this paper is simple, safe and effective.
