目的评价难治性腹泻型肠易激综合征（D-IBS）患者的生活质量并分析其相关影响因素。方法选择我院2011年2月～2013年6月门诊收治的符合罗马Ⅲ诊断标准的难治性D-IBS患者60例和其他功能性肠病（FBD）患者65例，使用肠易激综合征生存质量量表汉化版（ChlBS-QOL）、焦虑自评量表（SAS）以及抑郁自评量表（SDS）对患者进行问卷调查，同时建立患者的资料档案，对资料进行分析。结果观察组 D-IBS 患者 ChIBS-QOL8个维度和总条目的得分均明显低于对照组患者（P ＜0．05），两组比较差异有统计学意义，饮食限制为最低得分为（53．21±33．58）分，回归模型的决定系数 R ＝0．37，方差分析 F ＝19．93（P ＜0．01），回归模型有效。进入回归方程的变量为精神心理因素和文化程度。因此，精神心理因素与文化程度是影响 D-IBS 患者生存质量的两个重要因素。结论难治性 D-IBS 患者的生存质量明显降低，患者的精神心理因素以及文化程度对其生存质量有重要的影响。
목적평개난치성복사형장역격종합정（D-IBS）환자적생활질량병분석기상관영향인소。방법선택아원2011년2월～2013년6월문진수치적부합라마Ⅲ진단표준적난치성D-IBS환자60례화기타공능성장병（FBD）환자65례，사용장역격종합정생존질량량표한화판（ChlBS-QOL）、초필자평량표（SAS）이급억욱자평량표（SDS）대환자진행문권조사，동시건립환자적자료당안，대자료진행분석。결과관찰조 D-IBS 환자 ChIBS-QOL8개유도화총조목적득분균명현저우대조조환자（P ＜0．05），량조비교차이유통계학의의，음식한제위최저득분위（53．21±33．58）분，회귀모형적결정계수 R ＝0．37，방차분석 F ＝19．93（P ＜0．01），회귀모형유효。진입회귀방정적변량위정신심리인소화문화정도。인차，정신심리인소여문화정도시영향 D-IBS 환자생존질량적량개중요인소。결론난치성 D-IBS 환자적생존질량명현강저，환자적정신심리인소이급문화정도대기생존질량유중요적영향。
Objective To evaluate the life quality,related effect factors of refractory irritable bowel syndrome-diarrhea inducing type.Methods Sixty cases with diarrhea type(D-IBS)and 65 cases with other functional bowel(FBD)diagnosed refractory irritable bowel syndrome by Rome Ⅲ diagnostic criteria were recruited in outpatient department of our hospital between Feb.2011and Jun.2013.Question-naire investigation was proceeded including Life quality Scale Chinese Version(ChlBS-QOL),Self-Rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS),and collected data were analyzed.Results ChIBS-QOL8 dimensions and total scores in D-IBS patients were significantly lower than those in FBD patients(P <0.05),and the score of dietary restrictions was the lowest(53.21 ±33.58).The coefficient of determination R regression model was 0.37,ANOVA F was 19.93(P <0.01),therefore the regression model was valid.After analysis,variables into the regression equation included the degree of psychological and cultural factors,and both were important factors affected the life quality in D-IBS patients.Conclusion Of refractory irritable bowel syndrome,the life quality in patients with diarrhea was low,psychological and cultural factors have important influences on the degree of life quality.
