目的：研究多发性骨髓瘤患者外周血T淋巴细胞亚群与自然杀伤（NK）细胞的表达水平，探讨其细胞免疫功能与临床疗效的关系，并为判断、预测容易转归为难治患者提供具有参考价值的指标。方法：采用流式细胞术检测64例初治骨髓瘤患者化疗前及化疗中和全疗程结束后外周血T淋巴细胞亚群与NK细胞的表达水平。按化疗结果分完全缓解组、部分缓解组和难治组，与健康对照组比较。结果：①完全缓解组Th细胞、Ts细胞和NK细胞与对照组的差别无统计学意义。②部分缓解组Th细胞下降与对照组的差别有统计学意义。③难治组Th细胞、Ts细胞和NK细胞均呈明显下降，与对照组相比差别明显。④在最终死亡的三个患者，NK细胞百分比均在3%以下。结论：治疗后Th细胞、Ts细胞和NK细胞如果下降不明显，则提示该患者预后相对乐观，如果上述指标均明显下降，则提示预后不良。对于NK细胞严重降低的患者，提示预后极差。
목적：연구다발성골수류환자외주혈T림파세포아군여자연살상（NK）세포적표체수평，탐토기세포면역공능여림상료효적관계，병위판단、예측용역전귀위난치환자제공구유삼고개치적지표。방법：채용류식세포술검측64례초치골수류환자화료전급화료중화전료정결속후외주혈T림파세포아군여NK세포적표체수평。안화료결과분완전완해조、부분완해조화난치조，여건강대조조비교。결과：①완전완해조Th세포、Ts세포화NK세포여대조조적차별무통계학의의。②부분완해조Th세포하강여대조조적차별유통계학의의。③난치조Th세포、Ts세포화NK세포균정명현하강，여대조조상비차별명현。④재최종사망적삼개환자，NK세포백분비균재3%이하。결론：치료후Th세포、Ts세포화NK세포여과하강불명현，칙제시해환자예후상대악관，여과상술지표균명현하강，칙제시예후불량。대우NK세포엄중강저적환자，제시예후겁차。
Objective:To investigate the T cell subgroups and Natural Killer(NK)cell’s expression level of peripheral blood in Multiple myeloma patients to discuss the relationship of their cellular immune function and clinical curative effect. Methods:Flow cytometry was applied to detect T cell subgroups and NK cell’s expression level of peripheral blood in 64 MM patients with chemo-therapy and 58 normal controls. Results:①The ratio of T lymphocyte subgroup helper T cell,and suppressor T cell and NK cell in CR were close to the normal controls’.②The ratio of Th in the 35PR patients was significant differences compared to normal controls’.③The ratio of Th,Ts and NK in 23 refractory cases were statistically significant low than the controls’.④In the 3 died patients the ratio of NK was lower 3%. Conclusions: If the ratio of T lymphocyte subgroup Th cel,and Ts cell and NK cell had no significant differenc-es, the prognosis would be optimistic. In contrast, the patints would have unfavourable prognosis. Especially, if the ratio of NK cell had significant differences, death would be the undesired result.
