目的通过对乳腺癌超声数据分级系统的诊断价值及其征象与雌激素受体( ER)、孕激素受体( PR)以及人表皮生长因子受体2(Her-2)的相关性进行研究。方法选取以2012年5月至2014年5月就诊的68例乳腺癌患者作为研究对象,并根据乳腺超声图像报告和数据系统( BI-RADS)分级原则进行分级。术后对切除的乳腺肿瘤进行病理学分型分级,并对ER、PR以及Her-2阳性率进行测定,从而研究其与超声数据分级及征象的相关性。结果 BI-RADS 3级以良性乳腺肿瘤为主,BI-RADS 5级以恶性乳腺肿瘤为主。BI-RADS 3、4、5级的乳腺肿瘤诊断准确率分别为66.67%、72.22%和97.87%。BI-RADS 分级对乳腺肿瘤诊断的阳性预测值及阴性预测值分别为97.87%和66.67%。纵横比≥1、内部回声均匀以及微钙化的超声征象显示出较低的ER、PR阳性率和较高的Her-2阳性率( P<0.05)。结论乳腺癌的BI-RADS分级与病理学分型分级具有良好的相关性,对乳腺肿瘤诊断的准确性较高。并且超声征象与ER、PR以及Her-2具有一定的相关性,为预后以及治疗方案的选择提供了依据。

목적통과대유선암초성수거분급계통적진단개치급기정상여자격소수체( ER)、잉격소수체( PR)이급인표피생장인자수체2(Her-2)적상관성진행연구。방법선취이2012년5월지2014년5월취진적68례유선암환자작위연구대상,병근거유선초성도상보고화수거계통( BI-RADS)분급원칙진행분급。술후대절제적유선종류진행병이학분형분급,병대ER、PR이급Her-2양성솔진행측정,종이연구기여초성수거분급급정상적상관성。결과 BI-RADS 3급이량성유선종류위주,BI-RADS 5급이악성유선종류위주。BI-RADS 3、4、5급적유선종류진단준학솔분별위66.67%、72.22%화97.87%。BI-RADS 분급대유선종류진단적양성예측치급음성예측치분별위97.87%화66.67%。종횡비≥1、내부회성균균이급미개화적초성정상현시출교저적ER、PR양성솔화교고적Her-2양성솔( P<0.05)。결론유선암적BI-RADS분급여병이학분형분급구유량호적상관성,대유선종류진단적준학성교고。병차초성정상여ER、PR이급Her-2구유일정적상관성,위예후이급치료방안적선택제공료의거。
Objective The correlative analysis of ultrasound signs with ER,PR and Her-2 and the research on application of ultrasound BI-RADS grade in breast cancer were studied mainly in this paper. Methods 68 patients with breast cancer treated in our hospital from May 2012 to May 2014 were studied and graded according to the BI-RADS. Pathological types of breast cancer and the expression of ER,PR and Her-2 were performed after operation. The correlation with BI-RADS grade and ultrasound signs was studied. Results The diagnosis accuracy rate of breast tumor were 66. 67%,72. 22% and 97. 87% by BI-RADS 3,4 and 5 grade,respectively. The PPV and NPV in patients with BI-RADS 3~5 were 97. 87% and 66. 67%,respectively. The ultrasound signs of aspect ratio(≥1),uniform internal echo and microcalcification showed a low expression of ER,PR and higher expression of Her-2. Conclusion A signifcant correlation between BI-RADS grade and patho-logical types of breast cancer was obtained. The diagnosis accuracy rate of breast tumor was high. Furthermore,the correlation between ultrasound signs and ER,PR and Her-2 was good,and a evidence was provided for prognosis and treatment of breast cancer.
