目的探讨老年截石位手术病人在不同护理干预措施下放平下肢后血压的变化.方法收集2013年10月至2014年10月120例老年截石位手术病例均分为三组,每组40例.对照组接受常规护理,观察Ⅰ组接受下肢被动运动锻炼指导,观察Ⅱ组则给予医用弹力绷带护理.分别在术后不同时点记录三组患者血压变化.结果对照组患者t3～t1时段平均动脉压(3.98±0.91)mmHg,明显高于观察Ⅰ、Ⅱ组的(0.14±0.03)、(-0.41±0.08)mmHg (P＜0.05),观察组两组患者的血压无明显变化(P＞0.05).结论通过医用弹力绷带以及下肢被动运动训练两种护理方法可有效控制老年截石位手术患者血压波动程度,降低了截石位体位改变引发的低血压发生率,为围手术期患者身心健康与安全提供了一定保障.

Objective To investigate the effect of different nursing intervention on blood pressure after flatting lower limb in elderly patients undergoing lithotomy position operation.Methods 120 elderly patients undergoing lithotomy position operation from October 2013 to October 2014 were randomly divided into 3 groups,40 cases in each group.Control group received conventional nursing care,observation group Ⅰ received lower extremity passive exercise guidance,observation group Ⅱ received medical elastic bandage.Recorded blood pressure of 3 groups at different time points after operation.Results Mean arterial pressure (MAP) at t3-t1 period in control group was (3.98±0.91)mmHg,significantly higher than (0.14±0.03) mmHg in observation group Ⅰ and (-0.4 l±0.08)mmHg in observation group Ⅱ,with statistically significant differences (P＜0.05).There was no statistically significant difference in blood pressure between observation group Ⅰ and observation group Ⅱ (P＞0.05).Conclusion Medical elastic bandage and lower extremity passive exercise can effectively control blood pressure of elderly patients undergoing lithotomy position operation,reduce the incidence of hypotension caused by body position change,provides a guarantee for physical and mental health and safety of patients during perioperative period.
