目的:应用组织多普勒(TDI)测定右室Tei指数评价系统性红斑狼疮(SLE)的右室功能,并观察肺高压(PH)与右室Tei指数的关系,为临床早期干预治疗提供依据。方法:应用TDI对30例正常对照患者及30例SLE患者进行右室Tei指数测定,并测量Ea、Aa和Sa等参数,应用三尖瓣返流法估测肺动脉收缩压(PASP)。结果:(1)与对照组比较,SLE患者右室Tei指数显著增高,(P<0.05);(2)右室Tei指数升高的程度与PASP呈正相关(r=0.707, P<0.001);IRT及Am值明显增高,Em明显降低(P<0.05)。结论:(1)组织多普勒超声心动图测定的右室Tei指数有可能为SLE患者早期心功能改变的指标;(2)利用TDI测算右室Tei指数可较准确反映右室整体功能;(3)多普勒超声心动图可为临床非创伤性评估SLE患者肺高压提供一种新的方法。

Objective:To investigate the application of the Tei index,which reflects right ventricular (RV) function and to observe the clinical use of the Tei index for the patients with Systemic Lupus Erythematosus Diseases.Method:30 patients with SLE and 30 healthy controls were evaluated with routine echocardiography,all the patients internalized were not suffered with heart disease.The RV Tei index was measured by TDI.The peak velocities of diastolic and systolic tricuspid annulus in the right ventricular lateral wall (Ea,Aa,and Sa) were obtained.Result:(1)Compared with controls,the RV Tei index measured by TDI was significantly higher in the SLE group (P<0.05).(2) The RV Tei index in the SLE group measured by TDI was correlative with PASP (r=0.707,P<0.001﹚.Conclusion:(1)The Tei index is a novel and non-invasive method to assess RV systolic and diastolic function in patients with SLE.(2) TDI is a better method than conventional echocardiography in assessing RV function.(3)The Tei index is a sensitive and specific method to assess PH in the patients with SLE.
