目的:探讨冠脉造影患者高密度脂蛋白(high-density lipoprotein,HDL)亚类对冠状动脉狭窄程度影响的相关性。方法(1)抽取2013—2014年因疑诊冠心病(coronary heart disease,CHD)入住该院老年医学科、心内科行冠状动脉造影(coronary arteriography,CAG)的患者122例,经选择性冠状动脉造影确诊为CHD患者(至少有一支冠状动脉或分支狭窄>50%)96例,其中男62例,女34例,平均年龄(60.31±8.72)岁,未诊断CHD患者26例,男10例,女16例,平均年龄(59.00±13.51)岁;(2)抽取患者空腹12 h静脉血,选择双向电泳—免疫印迹检测法24 h内测定HDL亚类,对患者冠脉造影结果(采用Gensini评分)评价冠状动脉狭窄程度;(3)计量资料用均数±标准差(x ±s)表示,定性资料用χ2检验,两组比较采用独立样本的t检验, Logisitic回归分析检验与冠心病相关因素,两因素相关性采用线性相关分析。结果(1)单因素卡方、两样本独立t检验显示性别、吸烟史、糖尿病史、收缩压、体质指数(body mass index,BMI)、腰围(waist circumference,WC)、腰臀比(waist to hip ratio, WHR)、HDL、LDL、ApoA-I 、Lpa、Preβ1-HDL、HDL3b、HDL3c、HDL2b与冠心病有关,差异有统计学意义(P<0.05);(2)TG、Preβ1-HDL、HDL3b是冠心病危险因素,HDL2b是冠心病保护因素(P<0.05);(3)HDL2b与Gensini评分结果成负相关,Preβ1-HDL、HDL3b与Gensini评分结果成正相关。结论在临床诊断治疗中,HDL2b值有可能做为冠心病预测因素,Preβ1-HDL、HDL3b值可为预测和诊断CHD提供重要依据,冠心病的防治不仅要改善和促进胆固醇逆转运还要调节血脂,调整HDL亚类的构成比。

목적:탐토관맥조영환자고밀도지단백(high-density lipoprotein,HDL)아류대관상동맥협착정도영향적상관성。방법(1)추취2013—2014년인의진관심병(coronary heart disease,CHD)입주해원노년의학과、심내과행관상동맥조영(coronary arteriography,CAG)적환자122례,경선택성관상동맥조영학진위CHD환자(지소유일지관상동맥혹분지협착>50%)96례,기중남62례,녀34례,평균년령(60.31±8.72)세,미진단CHD환자26례,남10례,녀16례,평균년령(59.00±13.51)세;(2)추취환자공복12 h정맥혈,선택쌍향전영—면역인적검측법24 h내측정HDL아류,대환자관맥조영결과(채용Gensini평분)평개관상동맥협착정도;(3)계량자료용균수±표준차(x ±s)표시,정성자료용χ2검험,량조비교채용독립양본적t검험, Logisitic회귀분석검험여관심병상관인소,량인소상관성채용선성상관분석。결과(1)단인소잡방、량양본독립t검험현시성별、흡연사、당뇨병사、수축압、체질지수(body mass index,BMI)、요위(waist circumference,WC)、요둔비(waist to hip ratio, WHR)、HDL、LDL、ApoA-I 、Lpa、Preβ1-HDL、HDL3b、HDL3c、HDL2b여관심병유관,차이유통계학의의(P<0.05);(2)TG、Preβ1-HDL、HDL3b시관심병위험인소,HDL2b시관심병보호인소(P<0.05);(3)HDL2b여Gensini평분결과성부상관,Preβ1-HDL、HDL3b여Gensini평분결과성정상관。결론재림상진단치료중,HDL2b치유가능주위관심병예측인소,Preβ1-HDL、HDL3b치가위예측화진단CHD제공중요의거,관심병적방치불부요개선화촉진담고순역전운환요조절혈지,조정HDL아류적구성비。
Objective To investigate the relationship of high-density lipoprotein(HDL)subclasses with severity of coronary artery steno-sis in patients with Coronary angiography.Methods (1)A total of 122 consecutive patients underwent Coronary angiography were se-lected,because of suspected diagnosis of CHD was admitted to Gerontology Department,Vasculocardiology Department from 2013 to 2014 in our hospital,The confirmed diagnosis of CHD patients (At least have a coronary artery or branch stenosis>50%)96,62 males and 34 females,with an average of (60.31 ±8.72)years old,normal CAG findings patients 26,10 males and 16 females,with an aver-age of(59.00 ±13.51)years old;(2)Extraction in patients with venous blood fasting 12 hours,The contents of serum HDL subclasses were determined by two dimensional gel electrophoresis associated with immunodetection method within 24 hours.Through the evalua-tion of the results of coronary angiography in patients with coronary artery stenosis degree(by Gensini score);(3).measurement data with mean ±standard deviation (x ±s)and the qualitative data withχ2 test,Comparing the two groups using independent sample t test, Using logisitic regression test and coronary heart disease (CHD)related factors,The correlation between two factors using linear corre-lation analysis.Results (1 )Single factor chi-square,two independent samples t test showed that sex,smoking history,history of diabe-tes mellitus,systolic blood pressure,BMI,WC,WHR,HDL,LDL,ApoA-I,Lpa,Preβ1-HDL,HDL3b ,HDL3c ,HDL2b associated with coro-nary heart disease (CHD),The difference was statistically significant.(2)Smoking,systolic blood pressure,TG,Preβ1-HDL,HDL3b, HDL3c is coronary heart disease risk factors,HDL2b is coronary heart disease (CHD)protection factor.(3)HDL2b negative correlation with Gensini score,Preβ1- HDL,HDL3b are positively related with Gensini score.Conclusion In the clinical diagnosis and treatment, HDL2b possible as predictors for coronary heart disease,Preβ1-HDL,HDL3b numerical can provide important basis for prediction and di-agnosis of CHD,Coronary heart disease prevention is not only to improve and promote reverse cholesterol transport and regulating blood fat,but also regulate the HDL subclasses and their constituent ratio.
