【目的】探讨2型糖尿病(T2DM )患者血清25‐羟维生素D3(25‐OH‐D3)与胰岛细胞功能及胰岛素抵抗的相关性。【方法】入选T2DM 患者190例为观察组,同期健康体检者120例为对照组。根据糖尿病患者血清25‐OH‐D3值是否低于37.5 nmol/L ,分为低25‐OH‐D3亚组(LVD 组,n =80)及非25‐OH‐D3组(NLVD组,n =110)。测定受试者的空腹血糖(FPG)、空腹胰岛素(FINS)及糖化血红蛋白(HbA1c)水平,计算胰岛素抵抗指数(HOMA‐IR)及胰岛β细胞功能指数(HOMA‐β),并进行组间比较。【结果】观察组25‐OH‐D3水平低于对照组,观察组中LVD组的HOMA‐β显著低于NLVD组( P<0.05),FPG及HbA1c值较后者有上升趋势。相关分析提示25‐OH‐D3与H O M A‐β相关。【结论】糖尿病患者的25‐OH‐D3水平较健康人群低下,且与胰岛分泌功能下降有关。

【목적】탐토2형당뇨병(T2DM )환자혈청25‐간유생소D3(25‐OH‐D3)여이도세포공능급이도소저항적상관성。【방법】입선T2DM 환자190례위관찰조,동기건강체검자120례위대조조。근거당뇨병환자혈청25‐OH‐D3치시부저우37.5 nmol/L ,분위저25‐OH‐D3아조(LVD 조,n =80)급비25‐OH‐D3조(NLVD조,n =110)。측정수시자적공복혈당(FPG)、공복이도소(FINS)급당화혈홍단백(HbA1c)수평,계산이도소저항지수(HOMA‐IR)급이도β세포공능지수(HOMA‐β),병진행조간비교。【결과】관찰조25‐OH‐D3수평저우대조조,관찰조중LVD조적HOMA‐β현저저우NLVD조( P<0.05),FPG급HbA1c치교후자유상승추세。상관분석제시25‐OH‐D3여H O M A‐β상관。【결론】당뇨병환자적25‐OH‐D3수평교건강인군저하,차여이도분비공능하강유관。
[Objective] To explore the relationship between serum vitamin D level and islet βcell function and insulin resistance in type 2 diabetics .[Methods] Fasting serum level of 25‐hydroxyvitamin D3 [25‐(OH) D3 ] was measured in 190 patients with type 2 diabetes mellitus (T2DM ) and 120 healthy subjects .Fasting blood glucose (FBG ) ,fasting insulin (FINS ) ,βcell function index (HOM A‐β) and insulin resistance index (HOMA‐IR) were compared between type 2 diabetics with low 25‐(OH)D3 [25‐(OH)D3 <37 .5 nmol/L ,n=80] and those without ( n =110) .The correlations of serum 25‐(OH)D3 with FBG ,HbA1c ,insulin resist‐ance andβcell function were analyzed by Pearson's correlation analysis .[Results] The serum level of 25‐(OH) D3 was lower in type 2 diabetics than that in healthy subjects .Patients with a low level of 25‐(OH)D3 showed a lower HOMA‐β( P <0 .05)than those without ;and also showed higher FBG and HbA1c ( P >0 .05) than those without .The serum level of 25‐(OH)D3 was positively correlated with HOMA‐β.[Conclusion] Serum level of 25‐(OH)D3 is low in type 2 diabetics .And the deficiency of vitamin D in type 2 diabetes may be corre‐lated with decreased insulin secretion .
