目的：评价腰椎间盘切除联合椎弓根螺钉系统与椎间植骨融合治疗腰椎间盘突出症并退变性腰椎不稳的手术效果。方法随机选取2013年3~12月在第三军医大学第三附属医院行手术治疗的腰椎间盘突出症合并腰椎退变不稳患者36例，所有患者均采用腰椎间盘切除联合椎弓根螺钉系统与椎间植骨融合术进行治疗，评价临床疗效和手术满意度。结果患者手术治疗效果的优良率为94.44%，其中优24例(66.67%)，良10例(27.77%)，且所有患者均未出现严重并发症，患者自评满意度和医生评价满意度均为94.44%；同时半年以后植骨融合率亦为94.44%。结论腰椎间盘切除联合椎弓根螺钉系统和椎间植骨融合术治疗腰椎间盘突出症合并腰椎退变不稳效果显著，具有一定临床应用价值。
목적：평개요추간반절제연합추궁근라정계통여추간식골융합치료요추간반돌출증병퇴변성요추불은적수술효과。방법수궤선취2013년3~12월재제삼군의대학제삼부속의원행수술치료적요추간반돌출증합병요추퇴변불은환자36례，소유환자균채용요추간반절제연합추궁근라정계통여추간식골융합술진행치료，평개림상료효화수술만의도。결과환자수술치료효과적우량솔위94.44%，기중우24례(66.67%)，량10례(27.77%)，차소유환자균미출현엄중병발증，환자자평만의도화의생평개만의도균위94.44%；동시반년이후식골융합솔역위94.44%。결론요추간반절제연합추궁근라정계통화추간식골융합술치료요추간반돌출증합병요추퇴변불은효과현저，구유일정림상응용개치。
Objective To evaluate the surgical effect of lumbar disc excision combined with pedicle screw system and intervertebral bone graft fusion in the treatment of lumbar disc herniation complicated with degenerative instability. Methods Thirty-six patients with lumbar disc herniation and degenerative instability were randomly se-lected from the Third Affiliated Hospital of Third Military Medical University from March 2013 to December 2013. These patients were all treated by lumbar disc excision combined with pedicle screw system and intervertebral bone graft fusion. The clinical curative effect and the patients' satisfaction with the operation were analyzed. Results The surgical treatment effect was excellent in 24 cases (66.67%) and good in 10 cases (27.77%), with the excellent and good rate of surgical treatment was 94.44%. No severe complications were found. The patients' self-evaluated degree of satisfaction, the doctors'evaluated degree of satisfaction, and the bone graft fusion rate after six months was all 94.44%. Conclusion Lumbar disc excision combined with pedicle screw system and intervertebral bone graft fusion is effective for treating lumbar disc herniation complicated with degenerative instability, with certain clinical applica-tion value.
