目的报道1例应用Masquelet技术治疗儿童先天性胫骨假关节的手术方法和结果,并初步评价其疗效.方法回顾性报道2012年2月,我们用Masquelet技术治疗1例年龄5岁7个月男孩先天性胫骨假关节.查阅国内外相关最新文献报道.结果患儿随访2年2个月,胫骨长达17 cm骨缺损段完全成骨,且骨骼直径、骨皮质厚度均达到正常侧水平.目前遗留胫骨近端外翻和胫骨短缩畸形(胫骨近端外翻角约40°,胫骨较对侧短缩5 cm).膝关节活动范围正常,屈曲畸形完全矫正.患儿在矫形、增高支具的保护下行走活动无明显障碍.结论 Masquelet技术治疗大段骨缺损方法相对简便、效果优良,也适应于儿童病例,尤其是难治性先天性胫骨假关节.

Objective To explore the clinical efficacy of Masquelet technique for congenital pseudarthrosis of tibia (CPT) in children.Methods The authors reported the first case of treating CPT in children with Masquelet's procedure in China.And the relevant literature was reviewed and analyzed.Results The follow-up period was 2.1 years.A 17 cm bone defect achieved complete osteogenesis.And the diameter and thickness of tibial cortex bone were normal.There were some residual proximal tibial valgus and shortening of tibial malformation.The activity of knee joint was normal.Conclusions Masquelet technique is effective for long-bone defect with CPT in children.And it is ideal for refractory CPT after multiple operations.
