目的探讨鼻内镜下泪囊鼻腔开窗引流术中Crawford人工泪管置入的临床疗效.方法对2007年3月至2012年6月在呼伦贝尔市人民医院眼科就诊的98例(102只眼)慢性泪囊炎患者行鼻内镜下泪囊鼻腔造口术,所有患者术中制作鼻腔泪囊窗口,放置Crawford人工泪管,术后应用激素喷鼻;门诊随访时,在内镜下进行清创及泪道冲洗.结果 98例(102只眼)患者中4例泪囊炎复发,复发率占4.08％ (4/98).其中两例二次手术后泪囊炎治愈,冲洗泪道均通常,无溢泪,无流脓.两例未再复诊,成功率占97.96％ (96/98).结论鼻内镜下泪囊鼻腔引流术中Crawford管应用疗效满意.术中充分暴露泪囊、Crawford管应用是保证手术成功的关键;定期随访清创可促进黏膜健康愈合;及时发现并预防严重的瘢痕挛缩,可提高疗效.

Objective To investigate the curative effect of endoscopic dacryocystorhinostomy combined with Crowford-tube intubation.Methods The operative and postoperative data were collected in 98 chronic dacryocystitis patients (102eyes) who had undergone endoscopic dacryocystorhinostomy.During surgery,the mucosal flaps were preserved and no silicon intubation was adopted.The operative follow-up management included nasal corticosteroid spray endoscopic debrider and lacrimal duct irrigation.Results After the primary operation,4(3.92％) of 102 cases were recurrence.With reoperative of two failed cases,100(98.04％) of 102 cases were treated successfully.Conclusions The therapeutic effect of dacryocystorhinostomy on chronic dacryocystitis is content.The key factor contributing to the successful intervention includes complete exposure of the sac and preservation of mucosal flap technique.Closed follow-up,debrider and prompt dealing with stenosis might benefit for getting a higher operative success rate.
