目的:探讨MSCT及后处理技术在脑膜瘤术前评估中的价值。方法:应用256层CT对40例脑膜瘤患者行容积扫描,行MPR、MIP、CPR及VR显示脑血管,分析脑膜瘤血供情况、脑膜瘤与脑动脉之间的关系,并与手术对照。结果:CT后处理图像显示33例脑膜瘤有供血血管,其中颈外动脉(硬脑膜动脉)供血10例,颈内动脉(软脑膜动脉)供血9例,颈内外混合供血(硬脑膜血管、软脑膜动脉混合供血)14例;供血动脉数目1~6支不等。显示脑膜瘤周围动脉(脑组织供血动脉)36例,24例有分支血管向脑膜瘤供血、12例未见分支进入肿瘤;15例过路动脉受肿瘤包绕、狭窄,23例血管被推移。19例脑膜瘤显示引流静脉及其静脉窦汇入点。结论:MSCT容积扫描及其后处理技术能较全面显示脑膜瘤供血血管、引流静脉及其数目,并能显示肿瘤周围脑血管与肿瘤之间的关系,能减少手术所致的大出血、大面积脑梗死等严重并发症,对手术有指导意义。

Objective:To explore the value of MSCT and reconstruction techniques in preoperative evaluating meningioma. Methods:Forty patients suffered from meningioma were examined with 256-slice spiral CT. The data of all the cases were pro-cessed with multi-planar reconstruction,curve planar reconstruction,maximal intensity projection,volume render to show cerebral blood vessels. Feeding artery of meningioma and relation of tumor with cerebral artery were analysed and were collated with operation. Results:CT reconstructive images showed arteries feeding tumors in 33 patients,including 10 meningiomas were pro-vided by external carotid artery (dural artery),9 meningiomas were provided by internal carotid artery (pia mater artery),14 meningiomas were provided by both external carotid artery and internal carotid artery (dural artery and pia mater artery). The amount of arteries feeding meningiomas were from 1 to 6. CT reconstructive images showed vessels around meningiomas (from cerebral arteries) in 36 cases,which branches entered into meningiomas in 24 cases and no branches entered into meningiomas in 12 cases. The draining vein and the location of inflooding sinus were showed in 19 cases. Conclusions:MSCT and recon-struction techniques can perfectly display feeding artery,draining vein and mount of vessels of meningioma. Furthermore,it can display the relationship between meningioma and cerebral artery around meningioma with the vessels,which can decrease serious surgical complications such as massive cerebral haemorrhage or large-scale brain infarct and has clinical significance for direct-ing operation.
