目的：探讨治疗性抚触( TT)结合心理护理对甲状腺切除术患者应激反应的影响。方法选取2012年1月—2014年1月收治的择期甲状腺切除术患者64例为研究对象,根据随机数字表法分为观察组和对照组各32例。对照组采取常规手术护理,观察组在常规手术护理基础上给予TT结合心理护理干预。比较两组患者不同时点的心率( HR)、收缩压( SBP)、舒张压( DBP)及血糖( BG)水平变化；采用焦虑自评量表( SAS)和抑郁自评量表( SDS)比较两组心理状况；采用视觉模拟疼痛评分( VAS)比较两组患者术后疼痛情况。结果麻醉前对照组患者SBP为(127.4±10.6)mmHg,HR为(78.6±6.7)次/min,均高于观察组,差异有统计学意义( t 值分别为-2.424,-2.290；P <0.05)；术后2 h 对照组 SBP 为(123.7±11.8)mmHg,仍高于观察组,差异有统计学意义(t=-2.367,P<0.05)。术后1 d观察组SAS为(32.6±9.7)分,SDS为(31.2±10.1)分,均低于对照组,差异有统计学意义(t值分别为2.369,2.292；P<0.05)。观察组术后6 h疼痛VAS评分为(2.47±1.18)分,对照组为(3.84±1.26)分,差异有统计学意义(t=2.481,P<0.05)。结论 TT结合心理护理模式可缓解甲状腺切除术患者心理及生理应激反应,减轻术后疼痛,有利于患者术后康复。

목적：탐토치료성무촉( TT)결합심리호리대갑상선절제술환자응격반응적영향。방법선취2012년1월—2014년1월수치적택기갑상선절제술환자64례위연구대상,근거수궤수자표법분위관찰조화대조조각32례。대조조채취상규수술호리,관찰조재상규수술호리기출상급여TT결합심리호리간예。비교량조환자불동시점적심솔( HR)、수축압( SBP)、서장압( DBP)급혈당( BG)수평변화；채용초필자평량표( SAS)화억욱자평량표( SDS)비교량조심리상황；채용시각모의동통평분( VAS)비교량조환자술후동통정황。결과마취전대조조환자SBP위(127.4±10.6)mmHg,HR위(78.6±6.7)차/min,균고우관찰조,차이유통계학의의( t 치분별위-2.424,-2.290；P <0.05)；술후2 h 대조조 SBP 위(123.7±11.8)mmHg,잉고우관찰조,차이유통계학의의(t=-2.367,P<0.05)。술후1 d관찰조SAS위(32.6±9.7)분,SDS위(31.2±10.1)분,균저우대조조,차이유통계학의의(t치분별위2.369,2.292；P<0.05)。관찰조술후6 h동통VAS평분위(2.47±1.18)분,대조조위(3.84±1.26)분,차이유통계학의의(t=2.481,P<0.05)。결론 TT결합심리호리모식가완해갑상선절제술환자심리급생리응격반응,감경술후동통,유리우환자술후강복。
Objective To investigate the effect of therapeutic caress ( TC ) combined with mental nursing on stress response of patients with thyroid surgery. Methods A total of 64 patients, who underwent thyroid surgery in our hospital from January 2012 to January 2014, were randomly divided into observation group and control group, 32 patients in each group. The patients of control group were treated with normal surgery nursing while the patients of observation group were treated with TC combined with mental nursing on basis of normal surgery nursing. Heart rate (HR), systolic and diastolic blood pressure (SBP, DBP), and change of blood glucose ( BG) were compared;mental health assessed by self-rating anxiety scale ( SAS) and self-rating depression scale (SDS) scores before and after nursing intervention; the VAS pain score at 6h and 24h after operation were compared between the two groups. Results Before anesthesia in the control group, the SBP was (127.4 ±10.6)mmHg, HR (78.6 ±6.7) time/min higher than these of observation group (t = -2.424,-2. 290;P<0. 05); 2 hours after operation, SBP was (123. 7 ± 11. 8) mmHg, the same trend with above compared with observation group (t = -2. 367, P <0. 05). 1 day after openation SAS,SDS scores were (32. 6 ± 9. 7) and (31. 2 ± 10. 1) in the observation group lower than these of the control group ( t=2. 369, 2. 292;P<0. 05). 6 hours after operation in the observation group, the pain score of VAS was (2. 47 ± 1. 18) and (3. 84 ± 1. 26) in the control group (t =2. 481,P <0.05). Conclusions TC combined with mental nursing mode can effectively alleviate the psychological and physiological stress response in patients with thyroid surgery, obviously relieve the pain after operation, and thereby improve the postoperative rehabilitation.
