目的:探讨重型颅脑损伤患者早期营养支持的临床意义。方法回顾性分析在郑州市第一人民医院治疗的100例重型颅脑损伤患者的临床资料,其中20例早期给予肠内营养(EN)联合肠外营养(PN)营养支持(Ⅰ组);20例早期进行 PN,1周后过渡为 EN(Ⅱ组);20例早期单纯给予持续2周 PN(Ⅲ组);20例早期单纯给予 EN(Ⅳ组);20例给予延迟 EN(Ⅴ组)。观察五组胃肠功能恢复情况、营养状况以及免疫指标变化。结果Ⅳ组血清白蛋白、血红蛋白和外周血淋巴细胞总数在伤后第7天显著低于Ⅰ、Ⅱ、Ⅲ组(P ﹤0.05)。Ⅴ组伤后第7天和第14天各指标均显著低于Ⅰ、Ⅱ、Ⅲ组(P ﹤0.05)。结论早期给予颅脑损伤患者肠内营养支持结合肠外营养支持有利于改善预后。

목적:탐토중형로뇌손상환자조기영양지지적림상의의。방법회고성분석재정주시제일인민의원치료적100례중형로뇌손상환자적림상자료,기중20례조기급여장내영양(EN)연합장외영양(PN)영양지지(Ⅰ조);20례조기진행 PN,1주후과도위 EN(Ⅱ조);20례조기단순급여지속2주 PN(Ⅲ조);20례조기단순급여 EN(Ⅳ조);20례급여연지 EN(Ⅴ조)。관찰오조위장공능회복정황、영양상황이급면역지표변화。결과Ⅳ조혈청백단백、혈홍단백화외주혈림파세포총수재상후제7천현저저우Ⅰ、Ⅱ、Ⅲ조(P ﹤0.05)。Ⅴ조상후제7천화제14천각지표균현저저우Ⅰ、Ⅱ、Ⅲ조(P ﹤0.05)。결론조기급여로뇌손상환자장내영양지지결합장외영양지지유리우개선예후。
Objective To investigate the clinical efficacy of early nutrition on severe craniocerebral injury patients. Methods The clinical data of 100 patients with severe craniocerebral injury were retrospectively analyzed. Twenty cases were given early nutrition support of PN combined with EN. Twenty cases were given early nutrition support of PN,and changed to EN after 1 week. Twenty cases were given early nutrition support of PN. Twenty cases were given early nutrition support of EN. Twenty cases were given delay nutrition support of EN. The recovery of gastrointestinal functions,nutritional status and the change of immune index among the five groups were observed. Results Levels of Alb,Hb and peripheral blood lymphocytes of group Ⅳ at 7th day were lower than those of group Ⅰ,Ⅱ and Ⅲ(P ﹤ 0. 05). All index of group Ⅴ at 7th and 14th day were lower than those of group Ⅰ,Ⅱ and Ⅲ(P ﹤ 0. 05). Conclusion Early nutrition for severe craniocerebral injury patients is good for prognosis.
