目的:探讨肾功能不全对急性冠脉综合征患者远期预后的影响。方法选取260例急性冠脉综合征患者为研究对象,依据肾小球滤过率(eGFR)分为 eGFR 正常组和 eGFR 降低组。观察两组基础性疾病和严重心血管事件发生情况。结果 eGFR 降低组急性冠脉综合症患者合并高血压、糖尿病、缺血性疾病、高血脂等基础性疾病发生率均高于eGFR正常组,差异有统计学意义( P ﹤0.05);严重心血管不良事件发生率高于 eGFR 正常组,但差异未见统计学意义(P ﹤0.05)。结论急性冠脉综合征合并肾功能不全患者高血压、糖尿病等基础性疾病发生率高,极易发生心源性死亡等严重心血管不良事件,病死率高,远期预后差。

목적:탐토신공능불전대급성관맥종합정환자원기예후적영향。방법선취260례급성관맥종합정환자위연구대상,의거신소구려과솔(eGFR)분위 eGFR 정상조화 eGFR 강저조。관찰량조기출성질병화엄중심혈관사건발생정황。결과 eGFR 강저조급성관맥종합증환자합병고혈압、당뇨병、결혈성질병、고혈지등기출성질병발생솔균고우eGFR정상조,차이유통계학의의( P ﹤0.05);엄중심혈관불량사건발생솔고우 eGFR 정상조,단차이미견통계학의의(P ﹤0.05)。결론급성관맥종합정합병신공능불전환자고혈압、당뇨병등기출성질병발생솔고,겁역발생심원성사망등엄중심혈관불량사건,병사솔고,원기예후차。
Objective To investigate the effect of renal dysfunction on long term prognosis of patients with acute coronary syndrome. Methods Two hundred and sixty patients with acute coronary syndrome were chosen as the objects. According to glomerular filtration rate(eGFR),the patients were divided into normal eGFR group and reduced eGFR group. The basic disea-ses and serious cardiovascular events were observed. Results The incidences of acute coronary syndrome complicated with hy-pertension,diabetes,ischemic disease,hyperlipidemia and other basic diseases were higher in the reduced eGFR group than those in the normal eGFR group,there were significant differences(P ﹤ 0. 05). The rate of serious cardiovascular events in the reduced eGFR group was higher than that of the normal eGFR group,but there was no significant difference( P ﹤ 0. 05). Conclusion The incidence of hypertension,diabetes with other basic diseases in patients with acute coronary syndrome and re-nal insufficiency is high,and it is prone to sudden cardiac death and other serious cardiovascular adverse events. The mortality rate is high with poor prognosis.
