目的:观察右美托嘧啶在颅内肿瘤手术中的麻醉效果及安全性。方法将2012年3月~2014年3月于我院行颅内肿瘤手术的患者58例,其中对照组28例依次静脉注射咪达唑仑、异丙酚、舒芬太尼、维库溴铵,观察组30例在上述静脉注射麻醉的基础上则先于10 min内注射右美托咪定,后改为持续输注。观察和记录所有患者手术前、气管插管后5 min、瘤体分离时、手术结束时的收缩压、心率及颈内静脉血氧饱和度。结果对照组气管插管后5 min收缩压、心率高于观察组(P<0.05),观察组气管插管5 min、瘤体分离时、手术结束时颈内静脉血氧饱和度低于对照组(P<0.05)。结论右美托咪定应用于颅内肿瘤手术时,改变脑供氧状态,稳定血流动力学。

목적:관찰우미탁밀정재로내종류수술중적마취효과급안전성。방법장2012년3월~2014년3월우아원행로내종류수술적환자58례,기중대조조28례의차정맥주사미체서륜、이병분、서분태니、유고추안,관찰조30례재상술정맥주사마취적기출상칙선우10 min내주사우미탁미정,후개위지속수주。관찰화기록소유환자수술전、기관삽관후5 min、류체분리시、수술결속시적수축압、심솔급경내정맥혈양포화도。결과대조조기관삽관후5 min수축압、심솔고우관찰조(P<0.05),관찰조기관삽관5 min、류체분리시、수술결속시경내정맥혈양포화도저우대조조(P<0.05)。결론우미탁미정응용우로내종류수술시,개변뇌공양상태,은정혈류동역학。
Objective To observe the efficacy and safety of dexmedetomidine in treatment of patients with intracranial tumors. Methods 58 patients with intracranial tumor were selected. 28 cases in the control group were intravenous injection of midazolam,propofol,sufentanil,vecuronium in order. 30 cases in the observation group were intravenous anesthesia with dexmedetomidine 10 min earlier on the basis of the control group,than change the administration methods to continuous infusion. The systolic blood pressure,heart rate and blood oxygen saturation of jugular vein were recorded at the time of before and after operation, 5 min after tracheal intubation and separation of tumor body. Results The systolic blood pressure and heart rate of control group were higher than that of observation group at time of 5 min after tracheal intubation(P<0.05). The blood oxygen saturation of jugular vein of observation group was lower than that of the control group at the time of 5min after tracheal intubation,separation of tumor body,the end of operation(P<0.05). Conclusion Dexmedetomidine has the function of improving the state of cerebral oxygen supply and stabilizing hemodynamics when it is using for the patients with intracranial tumors.
