目的观察小牛血去蛋白提取物滴眼液治疗中重度干眼的效果.方法采用随机双盲交叉设计研究.对2013年9月至2014年4月于重庆医科大学附属第一医院眼表门诊确诊为中重度干眼的患者40例(80只眼),按检查的先后顺序随机分为A组(20例40只眼)和B组(20例40只眼).治疗第一阶段A组接受自体血清治疗,B组接受小牛血去蛋白提取物治疗;第一阶段结束后停药2周,作为洗脱期;第二阶段两组用药交换.分别在两个阶段的治疗前,治疗后2周、1个月各时间点检测并评定患者主客观表现评分.结果眼部症状视疲劳、干涩感、异物感、烧灼感、畏光、疼痛、眼表疾病指数(OSDI)评分、眼红、视力在A组和B组两种药物治疗后均较治疗前明显好转,差异有统计学意义(P＜0.05).客观体征荧光素钠染色评分、BUT、Sit在A、B两组治疗前后亦有明显改善,差异有统计学意义(P＜0.05).通过交叉设计方差分析比较以上主客观症状在两种药物间和两种给药顺序间差异无统计学意义(P＞0.05).结论小牛血去蛋白提取物滴眼液在治疗中重度干眼(尤其是中度干眼),对缓解眼部症状,促进角膜上皮修复,延长泪膜破裂时间均有明显疗效,其效果与自体血清相当.

Objective To observe the efficiency of calf blood extract eye drops on moderate to severe dry eye.Methods It was a two-stage cross-over design double-blind controlled study.Forty patients (80 eyes) diagnosed moderate to severe dry eye in Ocular Surface Clinic of The First Affiliated Hospital of Chongqing Medical University from September 2013 to April 2014 were chronologically divide into two groups (Group A,Group B,20 patients per group).Group A accepted treatment with autologous serum in the first stage,whereas Group B accepted treatment with calf blood extract eye drops.After 50 days of elution,the treatments of the two groups were exchanged.The signs and symptoms with different therapies were recorded at pre-therapy and post-therapy (2 weeks,1 month).Results Ocular dry eye symptoms including visual tiredness,dry and unsmooth sensation,foreign body sensation,burning sensation,photophobia,pain,ocular surface disease index score,redness,and visual acuity had acquired significantly improvement in both groups.The similar results obtained in Corneal FL,BUT and Sit.ALL of the ocular dry eye signs and symptoms mentioned before had not significantly acquired improvement between the two groups.Conclusions Calf blood extract eye drops has obvious efficiency on releasing ocular dry eye symptoms,promoting corneal epithelial cells repair,prolonging the breakup time of tear film,and is comparable to the aotologous serum eye drops in moderate-severe dry eye patients (especially moderate).
