Surgical reconstruction of blowout medial and inferior orbital walls%MEDPOR-TITAN implants
目的 研究MEDPOR-TITAN种植体在爆裂性眶内及下壁骨折整复术中的应用疗效.方法 对吉林大学第二医院眼眶病与眼整形科,自2013年10月至2014年4月就诊的85例(85只眼)爆裂性眶内及下壁骨折患者植入MEDPOR-TITAN种植体.采用下睑缘睫毛下切口联合泪阜结膜入路,充分暴露骨折的各个边缘,将嵌顿疝出的眶内组织(眼外肌及眶脂)充分还纳至眶内,然后将MED-POR-TITAN种植体依骨折面积及眶腔解剖形状修剪,填充于骨折缺损处,缝合骨膜及手术切口.结果 术后随访6个月,眼眶CT:骨折对位修复良好,MEDPOR-TITAN种植体固定良好、无移位、无排斥反应.术眼眼球内陷及眼球运动较术前明显改善,无视力下降、感染及其他不良反应.结论 爆裂性眶内及下壁骨折患者早期(伤后2~3周)施行MEDPOR-TITAN种植体修复,可以有效地矫正患者的眼球内陷,改善复视,且术后炎症反应轻,组织相容性好,在眼眶爆裂性眶内及下壁骨折整复术中是一种安全有效、值得推广的填充材料.
目的 研究MEDPOR-TITAN種植體在爆裂性眶內及下壁骨摺整複術中的應用療效.方法 對吉林大學第二醫院眼眶病與眼整形科,自2013年10月至2014年4月就診的85例(85隻眼)爆裂性眶內及下壁骨摺患者植入MEDPOR-TITAN種植體.採用下瞼緣睫毛下切口聯閤淚阜結膜入路,充分暴露骨摺的各箇邊緣,將嵌頓疝齣的眶內組織(眼外肌及眶脂)充分還納至眶內,然後將MED-POR-TITAN種植體依骨摺麵積及眶腔解剖形狀脩剪,填充于骨摺缺損處,縫閤骨膜及手術切口.結果 術後隨訪6箇月,眼眶CT:骨摺對位脩複良好,MEDPOR-TITAN種植體固定良好、無移位、無排斥反應.術眼眼毬內陷及眼毬運動較術前明顯改善,無視力下降、感染及其他不良反應.結論 爆裂性眶內及下壁骨摺患者早期(傷後2~3週)施行MEDPOR-TITAN種植體脩複,可以有效地矯正患者的眼毬內陷,改善複視,且術後炎癥反應輕,組織相容性好,在眼眶爆裂性眶內及下壁骨摺整複術中是一種安全有效、值得推廣的填充材料.
목적 연구MEDPOR-TITAN충식체재폭렬성광내급하벽골절정복술중적응용료효.방법 대길림대학제이의원안광병여안정형과,자2013년10월지2014년4월취진적85례(85지안)폭렬성광내급하벽골절환자식입MEDPOR-TITAN충식체.채용하검연첩모하절구연합루부결막입로,충분폭로골절적각개변연,장감돈산출적광내조직(안외기급광지)충분환납지광내,연후장MED-POR-TITAN충식체의골절면적급광강해부형상수전,전충우골절결손처,봉합골막급수술절구.결과 술후수방6개월,안광CT:골절대위수복량호,MEDPOR-TITAN충식체고정량호、무이위、무배척반응.술안안구내함급안구운동교술전명현개선,무시력하강、감염급기타불량반응.결론 폭렬성광내급하벽골절환자조기(상후2~3주)시행MEDPOR-TITAN충식체수복,가이유효지교정환자적안구내함,개선복시,차술후염증반응경,조직상용성호,재안광폭렬성광내급하벽골절정복술중시일충안전유효、치득추엄적전충재료.
Objective To explore the application effect of MEDPOR-TITAN implants in combined surgical reconstruction of blowout medial and inferior orbital walls.Methods From October 2013 to April 2014,85 patients (85 eyes) with blowout medial and inferior orbital walls adopted combined surgical reconstruction using MEDPOR-TITAN implants.The surgical repair was undergone via the joint lower limbus palpebralis and the transconjunctival incision.Fully exposed fractures each edge to obtain an adequate view of the fracture area,and the incarcerated hemia of soft tissues (extraocular muscle and orbital fat body) surrounding the orbit were subsequently restored and recovered.Then MEDPOR-TITAN implant was trimmed according to the fracture area and the anatomical shape of the orbital cavity and was inserted and fixed as a pad to fill up the defect of the orbital wall in the surgery of reconstruction.Finally,the periosteum and other surgical incision were sutured.Results Following up six months after surgery,the image of orbital CT scans displaed fracture counterpoint good repair.MEDPOR-TITAN implants were well fixed,with better results,no hypoglobus and no extrusion reaction.The enophthalmos and eye movement were significantly improved compared with the preoperative.No visual loss,no rejection or other adverse reactions occurred.Conclusions The patients with blowout fractures of orbital medial and inferior walls early (within 2-3 weeks of injury) treated through surgical reconstruction using MEDPOR-TITAN implants.The exophthalmos are well corrected; diplopia and the limit of eyeball movement are improved.The inflammation is mild after surgery,with better tissue compatibility,fewer complications.MEDPOR-TITAN implants is a good substitute material of bone graft and is much better than other artificial material.
