目的：研究SIRT1表达水平与乳腺癌患者CT征象的关系。方法研究纳入经病理诊断确诊的乳腺癌患者45例，收集患者乳腺癌组织和对应癌周正常组织标本，免疫组织化学法分析SIRT1的表达变化。利用CT行乳腺常规扫描，观察病灶形态、边缘、生长方式、钙化及坏死等直接征象和腋窝淋巴结转移等间接征象。分析SIRT1与乳腺癌患者CT表现的关系。结果乳腺癌组织中的SIRT1阳性表达率较高，乳腺癌患者的部分CT影像学特征与其SIRT1表达相关：SIRT1表达情况与乳腺癌病灶CT下的形态无关，SIRT1表达情况与病灶边缘形态有关，对于高SIRT1表达的患者，边缘毛刺征较为常见；从病灶生长分布形态来看，高SIRT1表的患者，其肿块和浸润弥漫分布的例数较多，肿块型的病灶直径较大。间接征象方面，钙化程度与SIRT1表达无关，SIRT1高表达的患者出现内部坏死的例数较多，更重要的是，高SIRT1表达的患者其腋窝淋巴结转移的例数较多。结论SIRT1可能参与乳腺癌的发生和发展，SIRT1检测与患者的CT像学征象有关，二者联合可提高乳腺癌的诊断价值。
목적：연구SIRT1표체수평여유선암환자CT정상적관계。방법연구납입경병리진단학진적유선암환자45례，수집환자유선암조직화대응암주정상조직표본，면역조직화학법분석SIRT1적표체변화。이용CT행유선상규소묘，관찰병조형태、변연、생장방식、개화급배사등직접정상화액와림파결전이등간접정상。분석SIRT1여유선암환자CT표현적관계。결과유선암조직중적SIRT1양성표체솔교고，유선암환자적부분CT영상학특정여기SIRT1표체상관：SIRT1표체정황여유선암병조CT하적형태무관，SIRT1표체정황여병조변연형태유관，대우고SIRT1표체적환자，변연모자정교위상견；종병조생장분포형태래간，고SIRT1표적환자，기종괴화침윤미만분포적례수교다，종괴형적병조직경교대。간접정상방면，개화정도여SIRT1표체무관，SIRT1고표체적환자출현내부배사적례수교다，경중요적시，고SIRT1표체적환자기액와림파결전이적례수교다。결론SIRT1가능삼여유선암적발생화발전，SIRT1검측여환자적CT상학정상유관，이자연합가제고유선암적진단개치。
Objective To analyse the correlation of SIRT1 expression with CT appearance in breast cancer patients. Methods This stduy selected 45 patients with pathological y diagnosed breast cancer. Tumor tissue samples and matched adjacent normal tissue samples were col ected and the SIRT1 expressions in tissue samples were analyzed by immunohistochemistry. CT appearance of al cases including lesions characters, calcification degree and necrosis degree were detected., also the lymph node metastasis status of patients were determined. Assay was performed to shown the relationship between expression changes of SIRT1 and CT appearance. Results Compared with normal tissue, in cancer tissue, SIRT1 expression was higher dramatical y, meanwhile, some of the CT appearance in patients were related to the level of SIRT1. In brief, There was no association between lesion shape and SIRT1 expression, however, the margin character of lesion was related to the SIRT1. It was more common that higher SIRT1 patients shown spiculation. Considering the growth charcters of lesions, we found that the lesions in most of higher SIRT1 patients were massive type or dif use type with a larger diameter. No evidence showed that SIRT1 expression was associated with calcification, but higher SIRT1 patients tended to show necrosis in lesions. More importantly, more lymph node metastasis was observed in higher SIRT1 patients. Conclusion There is definite correlation between SIRT1 expression and the CT appearance in breast cancer patients. SIRT1 could be used in combination with CT to diagnose breast cancer.
